Perioral wrinkles: Causes and remedies

Table of content
Perioral wrinkles may indicate a woman who is genuinely happy. However, the presence of wrinkles around the mouth, along with other facial wrinkles associated with the aging process, can lead to a loss of self-confidence in women. Nonetheless, understanding the various causes of perioral wrinkles and implementing proactive prevention strategies can lead to prompt improvement of these imperfections.

1. What are the potential causes of perioral wrinkles?

Perioral wrinkles can arise from multiple factors, including age, gender, genetics, sun exposure, tobacco use, and even individual beauty regimens.

Among these, the natural aging process is one of the primary and unavoidable causes of perioral wrinkle formation. As individuals age, they will likely observe the emergence of multiple lines and wrinkles on the forehead, under the eyes, between the eyebrows, and around the mouth. Due to the structural characteristics of the skin surrounding the mouth, women are more prone to developing wrinkles in this area compared to men.

Moreover, lifestyle factors such as excessive sun exposure and tobacco smoking can significantly increase the likelihood of wrinkle formation in general, and specifically wrinkle around the mouth. Tobacco use markedly heightens the risk of early perioral wrinkles due to increased muscle movement in the area compared to non-smokers. Additionally, there are various hypotheses suggesting that excessive kissing may also contribute to the formation of such wrinkles, although the cause of perioral wrinkles  is rarely acknowledged.

2. Common types of perioral wrinkles

There are three commonly observed types of perioral wrinkles around the mouth, including laugh lines, Marionette lines, and vertical lip lines.

  • Laugh lines: These extend from the lower corners of the nose to the corners of the mouth on either side. This line forms due to the loosening of the ligaments around the mouth, causing the skin to sag slightly.
  • Marionette lines: These wrinkles appear at the corners of the mouth and can give a slightly unhappy appearance, as they cause the mouth corners to droop. Such perioral wrinkles often arise due to collagen degradation in the skin, which results in sagging and the formation of folds around the mouth.
  • Vertical lip lines: These represent the most common wrinkles surrounding the mouth and are primarily caused by frequent lip movement. However, they can be exacerbated by smoking, leading to the colloquial term "smoker's lines."

3. What are the options for improving nasolabial folds?

When considering treatment modalities for nasolabial folds, the initial step is to accurately define the desired outcomes. Certain treatment options are indicated for the earlier stages of Marionette lines, whereas others are designated for patients exhibiting more severe and progressive wrinkles. Consequently, the treatment selected should align with the individual's needs and realistic goals, thus distinguishing the difference between successful treatment outcomes and the risks of treatment failure.

3.1. Facelift Surgery

One of the advanced aesthetic procedures aimed at alleviating nasolabial folds is facelift surgery.

Facelift surgery is fundamentally an invasive surgical procedure designed to address signs of skin aging, including wrinkles, crow's feet, sagging skin, and other manifestations of dermal aging. Concurrently, facial lifting is often performed alongside brow lifts, eyelid surgery, neck lifts, and other aesthetic interventions to significantly improve the appearance of aging skin, not solely concentrating around the mouth.

3.2. Dermal Fillers

Through injection, dermal fillers can assist in volumizing areas of the skin that have experienced volume loss, specifically at the nasolabial folds.

This process helps to minimize or eliminate the wrinkles, creating the foundation for a smoother and more youthful skin appearance.

3.3. Botox® Injections

Botox® is a derivative of botulinum toxin type A. This option is employed for both cosmetic and medical purposes, although it is more commonly known for its aesthetic application in preventing skin wrinkles and facial aging. Nonetheless, administering Botox® alone is not a suitable treatment for Marionette lines. Some evidence suggests that the combination of Botox and other injectable agents (e.g., hyaluronic acid and synthetic deoxycholic acid fillers) may function as a safe, effective, and less invasive alternative to traditional invasive surgical procedures.

3.4. Laser Skin Resurfacing

Laser skin resurfacing is a relatively novel and increasingly popular treatment for rejuvenating wrinkled and aged skin, thereby assisting in diminishing nasolabial folds.

This technique employs either ablative or non-ablative laser devices, depending on the severity of the wrinkles and other concurrent dermatological issues being addressed. For deeper folds at the corners of the mouth, an ablative laser may be utilized, while non-ablative lasers target minor imperfections.

In both techniques, lasers work by removing layers of damaged skin, revealing new, younger skin cells beneath. This method also enhances collagen production and promotes overall skin health improvements over time.

3.5. Light Therapy

Light therapy is a non-invasive treatment employed to address various skin conditions utilizing different wavelengths of LED light. This treatment serves as an adjunctive strategy to enhance the effectiveness of other techniques and is not utilized as an independent therapeutic method.

Nevertheless, the efficacy of light therapy will depend on the specific color applied, likely relating to red LED light therapy (the option that penetrates the skin most deeply, promoting new cell regeneration and improving aesthetics of nasolabial folds).

3.6. Chemical Peeling

Aimed at improving skin appearance, chemical peeling is a resurfacing technique utilized to remove layers of damaged skin.

This procedure involves the application of a solution to the patient's facial skin to exfoliate at a light, medium, or deep level, depending on the severity of the skin lines. After the exfoliation, the solution and exfoliated skin will slough away. This exfoliation process encourages cellular turnover, resulting in fresher and smoother skin, thereby diminishing nasolabial folds.

3.7. Microneedling

Microneedling, also known as collagen induction therapy, is a technique used to treat fine wrinkles, scars, enlarged pores, and to foster smoother and healthier skin.

Microneedling involves repeatedly puncturing the skin with numerous small needles. This process stimulates new cell production and encourages collagen proliferation, filling deeper wrinkles and reducing fine lines.

4. How to Prevent Nasolabial Folds?

For perioral wrinkles, the most effective approach is preventive treatment. The human body tends to better protect against potential issues rather than recover from conditions post-occurrence. Some of the optimal skin care choices for preventing perioral wrinkles include regular moisturization and appropriate makeup removal tailored to individual skin types. It is advisable for individuals to begin their skincare routine as early as possible to ensure optimal protection against the formation of wrinkles.

To care for the skin effectively, it is essential to remember that moisturizers, sunscreens, anti-aging creams, cleansers, and other skincare treatments should be utilized for the purpose of preventing aging. Once perioral wrinkles have formed, reliance solely on skincare or anti-aging products will be inadequate. However, by consistently maintaining an appropriate skincare regimen, individuals can optimally mitigate the speed at which wrinkles develop.

In summary, as age increases, the formation of new wrinkles, as well as the deepening of existing wrinkles, becomes inevitable, particularly concerning perioral wrinkles. Therefore, dedicating time to appropriate skincare is crucial for preventing skin aging at a younger age, and avoiding modifiable risk factors associated with perioral wrinkles represents ideal measures for preserving youthful aesthetics over the long term.
