1. What is Daxotel?

Mechanism of action of the drug:

2. Dosage and usage

2.1. How to use

The drug is prepared as an injection - 20mg/0.5ml for intravenous use, which can be used alone or in combination with other chemotherapy drugs.

2.2. Amount

The usual dose should be used as an infusion of 100 mg/m2 over 1 hour. Before infusion 1 day, the patient should take dexamethasone 8 mg x 2 times / day x 3 - 5 days. For patients with liver failure, it is necessary to reduce the dose of the drug compared to the usual dose, the specific dose should consult the doctor. For patients with grade 3 or 4 diarrhea after using docetaxel, the dose should be reduced to about 20%. Note:
If the patient has a decreased neutrophil count accompanied by fever or a neutrophil count < 500/mm3 for more than 1 week or has severe peripheral neuropathy, dosage adjustment is required. , refer to the dose of 75mg/m2. If the above reactions persist after taking the drug, the patient should continue to reduce the drug dose to 55mg/m2 or stop treatment.
For patients with renal impairment may not need to adjust the dose of the drug, because there have been studies showing that renal failure is not an influencing factor and it is necessary to adjust the dose of docetaxel.

3. Contraindications

Docetaxel is not indicated for use in patients with neutrophil counts below 1500 cells/uL. The drug should not be used in patients with a history of severe hypersensitivity reactions to docetaxel or polysorbate 80. The drug should be used with caution in the elderly over 65 years of age, because these cases are often prone to unwanted side effects. Docetaxel should not be used in patients with severe hepatic impairment. Use with caution in pregnancy: According to available clinical data, docetaxel appears to be safe during pregnancy if used during the third trimester. the second and third trimesters, however, this data is uncertain, so before using the drug, patients should consult their doctor to weigh the benefits and side effects that the drug brings and have the right dose of medication. Breast-feeding: It is not known whether this medicine passes into breast milk. However, to avoid possible risks to the infant, breast-feeding while using this medicine and for 1 week after stopping treatment is not recommended. There are limited trials of docetaxel in children, so the safety of docetaxel for use in patients younger than 16 years of age has not been established. Before using docetaxel, you should tell your doctor or pharmacist your medical history, especially of liver problems, lung problems (such as pulmonary effusion), problems with heart problems (such as congestive heart failure), a weak immune system (such as leukopenia), blood problems (such as anemia, thrombocytopenia), blood pressure problems.

4. Side effects

The most common side effects include hair loss, cytopenias (low blood cell counts), numbness in the limbs, shortness of breath, nausea, vomiting, and muscle pain. In some cases, vomiting and nausea can be more severe. There may be a reaction of pain or swelling at the injection site, diarrhea, excessive lacrimation, fatigue, dizziness, drowsiness, intoxication, constipation, and loss of appetite. Other serious side effects include allergic reactions, swelling of the hands and feet, unexplained weight gain, tingling in the hands or feet, muscle or joint pain, persistent fatigue, eye pain, irregular heartbeat, stomach pain, severe diarrhea, diarrhea with blood or mucus, severe headache. Side effects are more common in people with liver problems. Hematological side effects include leukopenia at 95.5%, anemia in 90.4%, fever in leukopenia 11.0%, and thrombocytopenia in 8.0%. Toxic death accounted for 1.7% of the 2045 patients, and 9.8% of the patients had additional morbidity in patients with liver dysfunction. Rare cases of gas poisoning caused by taxanes. However, 1–5% of patients receiving docetaxel may experience severe gas toxicity. 5. Drug interactions

Drug interactions can lead to changes in the effect of drugs or increase drug reactions when used in combination with Daxotel with other drugs. Cisplatin combined with daxotel increases the risk of neurodevelopmental retardation. Concomitant use with Cyclosporine, dalfopristin, erythromycin, itraconazole, ketoconazole, quinupristin, terfenadine, troleandomycin may increase the risk of other serious reactions such as: risk of docetaxel toxicity, anemia, leukopenia, thrombocytopenia, fever, diarrhea. Combination with doxorubicin hydrochloride increases the risk of cholestatic jaundice and pseudomembranous colitis. Co-administration with doxorubicin hydrochloride liposomes increased doxorubicin exposure. Combination with certain vaccines such as Bacillus of Calmette and Guerin, measles, mumps, polio, rotavirus, rubella, smallpox, typhoid, varicella, malaria, jaundice increases the risk of vaccine-induced infection living. Administration with thalidomide increases the risk of venous thrombosis. The dose of docetaxel may need to be increased when it is co-administered with anticonvulsants, which cause inhibition of CYP3A4. Cisplatin has in some cases been shown to reduce docetaxel clearance by up to 25%.
