Telmisartan is the active ingredient in Teli 40mg, which is an orally administered, specific angiotensin II receptor (AT1) receptor antagonist.
Teli 40mg is rapidly absorbed, although the amount absorbed is variable. The absolute bioavailability of telmisartan is about 50%. When administered with food, the area under the plasma concentration-time curve (AUC) for telmisartan can be reduced from 6% (at a dose of 40 mg) to about 19% (at a dose of 160 mg). Plasma concentrations of telmisartan taken with or without food after 3 hours were similar. AUC decreased slightly, so it did not reduce the effectiveness of treatment.
Gender will make a difference in plasma concentrations. Peak plasma concentrations (Cmax) are increased about 3-fold and AUC is increased about 2-fold in women compared with men. However, it does not affect the efficiency.
Telmisartan is bound to most plasma proteins (> 99.5%). Mainly with albumin and alpha-1 acid glycoprotein. The mean steady-state volume of distribution is about 500 L.
Telmisartan will be metabolized by conjugation with glucuronide. This metabolite has no pharmacological activity.
Telmisartan is kinetically destroyed by the quadratic equation. The terminal half-life is over about 20 hours. Peak plasma concentrations and area under the plasma concentration-time curve (AUC) increased disproportionately to the dose.
After oral or intravenous administration, telmisartan is almost completely eliminated in the feces. The majority is excreted unchanged. Total urinary excretion is less than 2% of the dose. Total plasma clearance is high (approximately 900 ml/min) compared with hepatic blood flow (approximately 1500 ml/min).
Patients with hepatic impairment: Several pharmacokinetic studies in patients with hepatic impairment have shown an increase in bioavailability up to nearly 100%. Therefore, the half-life is unchanged in patients with hepatic impairment. Patients with renal impairment: Plasma concentrations are lower in patients with renal impairment on dialysis. Telmisartan is associated with high plasma protein in patients with renal failure and does not pass the membrane during hemodialysis. The half-life is therefore unchanged in patients with renal impairment. Elderly: The pharmacokinetics of telmisartan do not differ between the young and the elderly.

2. Dosage and how to use Teli 40


Dosage and how to use Teli 40 can refer to the following:
For adults: Use 40mg x 1 time/day. The dose can be increased to 80mg x 1 time/day. Use as monotherapy or in combination with thiazide diuretics. Dosage does not need to be adjusted in the elderly or in renal failure. People with mild & moderate hepatic impairment: Use up to 40 mg/day.

3. Contraindications Teli 40


Contraindicated in all of the following cases:
People with hypersensitivity to the components of the drug. Women in the second & third trimesters of pregnancy & while breastfeeding. Patients with biliary obstruction or severe liver failure.

Drug interactions can occur, so it is necessary to monitor the concentration of digoxin when co-administered.

5. Teli 40 . side effects


The frequency of adverse events reported with telmisartan was 41.4% which was similar to that of placebo (43.9%) in placebo-controlled trials. At present, there is no relationship between adverse events and the dose, gender, age or race of the patients.
The adverse events listed below were cumulative from all clinical trials that included 5788 hypertensive patients treated with telmisartan.

Bone, Connective Tissue and Musculoskeletal System Disorders: Arthralgia, back pain, leg pain, myalgia, leg cramps, tendinitis-like symptoms. In addition, since Teli 40mg drug was introduced on the market, cases such as erythema, pruritus, syncope, bradycardia, tachycardia, insomnia, depression, vomiting, low blood pressure, dyspnoea, hyperthyroidism basophils, thrombocytopenia, weakness and poor performance have been reported very rarely.
As with other angiotensin II receptor antagonists, isolated cases of edema, urticaria and other associated symptoms have also been reported.
