Patients with renal impairment with creatinine clearance between 10-25ml/min: 500-1000mg for the starting dose and 500mg for the maintenance dose, each dose taken 24 hours apart. People with renal failure have creatinine clearance from 25 - 50ml/min: Use 500 - 1000mg for the starting dose and 500mg for the maintenance dose, each dose taken 12 hours apart. In case of missed dose of Zinextra: Users should use Zinextra regularly to get the best effect. However, if you miss a dose, skip the missed dose and continue with the next dose exactly as directed by your doctor.
