What is Newcilotal?


Newcilotal with Tramadol HCl 37.5mg; Acetaminophen 325mg and excipients are just enough to relieve moderate to severe pain.
Tramadol is a central analgesic. At least two mechanisms occur, binding of parent and active metabolite (M1) to the Mu-opioid receptor and mild inhibition of norepinephrine and serotonin reuptake.
Acetaminophen is another central pain reliever. The exact mechanism and site of its analgesic action are not clearly defined. When evaluated in laboratory animals, the combination of tramadol and paracetamol showed synergistic effects.
Newcilotal is absorbed immediately after oral administration, almost completely and in the small intestine. Peak plasma concentrations of Newcilotal are reached within 1 hour.
About 30% of the drug is excreted in the urine as unchanged drug, while about 60% of the drug is eliminated as metabolites. The major metabolites are demethylation at the N- and O- positions or conjugation with glucuronide or sulfate in the liver.

Precautions while using Newcilotal


Use caution when using the drug because of the risk of convulsions when tramadol is used concurrently with SSRIs (serotonin reuptake inhibitors), TCAs (tricyclic compounds), Opioids, MAOIs, sedatives or drugs that lower the seizure threshold; or in patients with epilepsy, in patients with a history of seizures, or in patients at risk for seizures. Use with caution, as there is a risk of respiratory depression in patients at risk of respiratory failure; use high doses of Newcilotal with local anesthetics, anesthetics, and alcohol. Caution should be exercised when using Newcilotal concomitantly with drugs that depress the central nervous system such as alcohol, opioids, local anesthetics, anesthetics, hypnotics and sedatives. Newcilotal should be used with caution in patients with increased intracranial pressure or head trauma. Use with caution in patients addicted to opiates, as it can cause relapse. Use with caution in patients with chronic alcoholism because of the risk of hepatotoxicity. For patients with creatinine clearance less than 30 ml/min, a dose of no more than 2 tablets is recommended every 12 hours. Use with caution in patients with severe hepatic impairment.
