Uses of Carhurol


Carhurol belongs to the group of cardiovascular drugs, containing the main ingredient Rosuvastatin, mainly produced in the form of 2 concentrations of Carhurol 10 and Carhurol 20. The drug is used in the treatment of blood lipid lowering.

1. What are the effects of Carhurol?

Carhurol has the main ingredient Rosuvastatin, which is a selective and competitive inhibitor of HMG-CoA reductase, the enzyme that catalyzes the conversion of 3-hydroxy-3-methylglutaryl coenzyme A to mevalonate (a precursor of cholesterol), from It has the effect of reducing lipids in the blood.
In addition, the main site of action of Rosuvastatin is the liver, the target organ of cholesterol lowering. Rosuvastatin increases the number of LDL receptors on the surface of hepatocytes, thereby increasing the absorption and catabolism of LDL, inhibits the synthesis of VLDL in the liver, thereby reducing the components of VLDL and LDL.
Rosuvastatin reduces levels of LDL- cholesterol, total cholesterol and triglycerides; increase HDL cholesterol. The drug also decreased ApoB, non-HDL-C, VLDL-TG and increased ApoA-I; reduced the ratio of LDL-C/HDL-C, total C/HDL-C, non-HDL-C/HDL-C and ApoB/ApoA-I.
Regarding the pharmacokinetics, peak plasma concentrations of Rosuvastatin are reached approximately 5 hours after oral administration. The drug is widely distributed mainly in the liver, which is the site of cholesterol synthesis and clearance of LDL-C. Rosuvastatin is poorly metabolised, is eliminated mainly (90%) as unchanged drug in the faeces, and the remainder is excreted in the urine. Carhurol is usually indicated in the following cases:
Primary hypercholesterolemia; Mixed dyslipidemia; Homozygous familial hypercholesterolemia.

2. Dosage of the drug Carhurol


Carhurol can be taken once at any time of the day, regardless of the meal. Depending on the subject and the treatment goal, the dose of Carhurol will be different, specifically as follows:
Start with 5-10mg/day, if necessary, the dose can be adjusted every 4 weeks. The 40 mg dose should only be used when severe hypercholesterolemia at high risk of cardiovascular disease is not achieved at the 20 mg dose. Patients should be monitored regularly when receiving a dose of 40 mg. Contraindications of Carhurol include:
Patients with hypersensitivity to any component of Carhurol; Liver disease develops including unexplained persistent elevations in serum transaminases and when transaminases are elevated more than 3 times the upper limit of normal; Severe kidney failure; Myopathy; Patient is taking Cyclosporin; Pregnant or lactating patients, women can become pregnant without using appropriate contraception.

3. Side effects of the drug Carhurol:


Adverse reactions reported with Rosuvastatin were generally mild and transient. In some patients when using Carhurol, side effects may occur such as:
Immune system disorders; Headache, dizziness; Itching, rash and urticaria; Myalgia, myopathy, rhabdomyolysis; Attenuate;

4. Be careful when using Carhurol


Some general notes when using Carhurol include:
Caution when using Statins for patients with risk factors for muscle damage because it can cause harmful reactions to the body such as atrophy, myositis especially in patients with high risk factors such as over 65 years old, with uncontrolled hypothyroidism, kidney disease. Patients should be treated immediately when there are signs of muscle pain, fatigue, fever, dark urine, nausea, vomiting during the use of Carhurol. The risk of myopathy is increased if Carhurol is used concomitantly with Cydossporin, fibric acid derivatives (including Gembifrozil), nicotinic acid, macrolide antibiotics, Azol-class antifungals or protease inhibitors. Patients with elevated transaminase levels should be monitored until the abnormality resolves. If elevation of ALT or AST persists more than 3 times the upper limit of normal, the dose should be reduced or the drug discontinued. Carhurol should be used with caution in patients who drink heavily or have a history of liver disease. Check creatine kinase (CK) levels if levels are significantly elevated above 5 times the upper limit of normal and treatment with this drug should not be initiated. When taking high doses of Carhurol (40mg) sometimes proteinuria may occur, renal function should be monitored and evaluated during high doses of the drug. When amiodarone is used in combination, the dose should not exceed 20 mg/day. Use caution when using Carhurol while driving or operating machinery. Carhurol is contraindicated for pregnant and lactating women.

5. Carhurol drug interactions


Co-administration of Carhurol with vitamin K antagonists (Warfarin) may increase the INR. Co-administration of Carhurol with Gemfibrozil may result in a doubling of Rosuvastatin concentrations and an increased risk of myopathy. Co-administration of Carhurol with Liponavir or Ritonavir also increases concentrations of Rosuvastatin. Oral suspensions of antacids containing magnesium and aluminum hydroxide have been shown to reduce plasma concentrations of Rosuvastatin by approximately 50%, which is less pronounced when taken 2 hours apart. Erythromycin reduces concentrations of Rosuvastatin, this interaction may be because Erythromycin increases intestinal motility. Taking Rosuvastatin with oral contraceptives such as Norethisterone and Ethinylestradiol increases the bioavailability of oral contraceptives. Rosuvastatin is not an inhibitor or inducer of cytochrome P450 enzymes, so there is no interaction with fluconazole or ketoconazole. Concomitant use of Carhurol with the following drugs increases the risk of muscle damage: other fibrinolytic drugs, high-dose Niacin, Colchicine. Concomitant use of lipid-lowering statins with drugs for HIV and hepatitis C may increase the risk of muscle damage, most seriously rhabdomyolysis leading to renal failure, which can be fatal. . When co-administering Rosuvastatin with Atazanavir, Atazanavir + Ritonavir must limit the dose of Rosuvastatin to a maximum of 10 mg/day.
