1. Uses of Urostad 40


What is Urostad 40? Urostad 40 has the main diuretic ingredient Furosemide 40mg. Furosemide belongs to the group of diuretics, is a sulfonamide derivative with strong, rapid and dose-dependent effects.
Urostad 40 acts in the ascending limb of the loop of Henle, so it is classified as a loop diuretic. The diuretic effect of the drug Urostad 40mg is strong, so it has the effect of lowering blood pressure, but it is usually weaker than other groups of drugs. In patients with pulmonary edema, the active ingredient Furosemide causes an increase in venous volume, thereby reducing left ventricular preload before the diuretic effect becomes apparent.
Urostad 40 is indicated for treatment in the following cases:
Acute pulmonary edema; Water retention, edema due to cardiovascular disease, liver and kidney disease and some other causes; Urostad 40 supports the treatment of hypertension when accompanied by kidney damage; Urostad 40 treats hypercalcemia. In contrast, the product Urostad 40mg is contraindicated for use in the following subjects:
People with a history of allergy to furosemide or other sulfonamide derivatives (eg, sulfonamides to treat diabetes); Do not use Urostad 40 for patients who are in a state of hepatic pre-coma, hepatic coma; Urostad 40 should not be used in patients with anuria or renal failure due to nephrotoxic or hepatotoxic drugs.

2. Dosage of the drug Urostad 40mg


Edema due to:
The starting dose is 40mg/day (1 Urostad 40 tablet), then the dose can be adjusted if necessary and depending on the patient's response; Patients with mild edema can take 1⁄2 Urostad 40 tablets/day or 1 Urostad 40 tablet taken every other day; In some cases of severe edema, the dose can be increased to 80mg (2 Urostad 40mg tablets) or higher, divided into 1 or 2 times a day and can be increased to a maximum dose of 600 mg/day; In children, the usual oral dose of Furosemide is 1-3 mg/kg/day and up to a maximum of 40 mg/day. Hypertension:
Urostad 40 is not the main drug in the treatment of hypertension, requiring combination with other antihypertensive drugs to treat hypertension in people with correct indications; The oral dose of Furosemide is 40-80 mg/day (1-2 Urostad 40 tablets) used alone or in combination with other antihypertensive agents. Hypercalcemia:
The recommended dose is 120mg/day (3 Urostad 40 tablets) orally once or divided into 2-3 times. The elderly are more sensitive to the side effects of Urostad 40 despite the same doses used in adults.

3. Side effects of the drug Urostad 40mg


When using Urostad 40, patients are at risk of unwanted effects (ADRs), but this problem mainly occurs when treated at high doses (accounting for 95% of cases with adverse reactions). ).
Some common side effects of Urostad 40:
Decreased circulating volume when using high doses; Orthostatic hypotension; Hypokalemia, hyponatremia and hypomagnesaemia; Decreased blood calcium; Hyperuricemia; Alkalosis due to hypochloremia. Some uncommon and rare adverse effects of the drug Urostad 40: Gastrointestinal disturbances causing nausea, vomiting; decrease in the number of white blood cells and platelets, agranulocytosis; paresthesia; tinnitus, reversible hearing loss (when using high doses of furosemide).

4. Some cautions when taking Urostad 40mg


Caution should be exercised when prescribing Urostad 40 to patients with prostate enlargement or difficulty urinating because Furosemide may prompt the patient to urinate.
Urostad 40 contains the excipient Lactose, therefore should not be used in patients with hereditary problems such as galactose intolerance, total lactase deficiency or glucose-galactose malabsorption.
It is recommended that patients taking Urostad 40 be careful when driving or operating complex machinery because the drug can cause orthostatic hypotension and headache.
Thiazides, thiazide diuretics and loop diuretics (eg Urostad 40) all cross the placental barrier to reach the fetal circulation and can lead to fluid and electrolyte disturbances. With thiazides and derivatives, many cases of neonatal thrombocytopenia have been reported and the risk is similar to that of loop diuretics such as furosemide and buretamide. Therefore, in the last 3 months of pregnancy, Urostad 40 should only be used when there is no alternative medicine, and indicated at the lowest dose for the shortest time.
Use of Urostad 40 during lactation may lead to the risk of inhibiting lactation, so in these cases, breastfeeding should be discontinued.

5. Drug interactions of Urostad 40mg


Urostad 40 combined with Cephalothin or Cephaloridin increases nephrotoxicity; Lithium salts: Urostad 40 increases blood lithium levels and can cause toxicity, so this combination should be avoided if blood lithium cannot be monitored closely; Aminoglycoside antibiotics when used concurrently with Urostad 40 increase ototoxicity and nephrotoxicity, so concurrent use should be limited; Cardiac glycosides may increase hypokalemic toxicity associated with Urostad 40, which requires frequent monitoring of serum potassium levels and electrocardiograms; Corticosteroids will cause increased excretion of potassium in the blood and worsen when used with Urostad 40; Urostad 40 combined with antidiabetic drugs increases the risk of hyperglycemia, so patients should be monitored and the dose of antidiabetic drugs adjusted accordingly; Non-depolarizing muscle relaxants: Urostad 40 will increase the muscle relaxant effect; Anticoagulants: Urostad 40 increases the anticoagulant effect, even causing coagulation disorders; Cisplatin combined with Urostad 40 leads to increased ototoxicity, so this combination should be avoided; Antihypertensives when administered with Urostad 40 potentiate their antihypertensive effect and require dose adjustment as needed, especially angiotensin-converting enzyme inhibitors.
