Uses of Velcade

1. What does Velcade do?


Bortezomib is an inhibitor of the reversibility of chymotrypsin-like activity on the 26S proteasome in mammals. The 26S proteasome is a large protein complex that degrades the ubiquitin protein. Ubiquitin-proteasome exists in the body and plays a key role in regulating the concentration of specific intracellular proteins, thereby helping to ensure endothelial stability within cells. Mechanism of inhibition of the 26S proteasome by blocking target proteolysis and possibly affecting intracellular endothelial pathways. Thus, inhibition of normal endothelial homeostasis can lead to cell death. Several trials have confirmed that Bortezomib is cytotoxic to various types of cancer cells in vitro.

2. Indications for the use of velcade


Velcade is indicated in the following cases:
Patients with multiple myeloma are prescribed parenteral administration. Patients with mantle cell lymphoma who have received at least 1 previous course of treatment.

3. How to use velcade


Before performing intravenous injection with Velcade, the drug must be reconstituted with 3.5ml of 0.9% isotonic NaCl solution or used USP standard NaCl solution for injection. Products to be reconstituted should be colorless, transparent. Otherwise, they should be visually inspected for particulate matter and discoloration prior to use in both the vial and solution for injection. If any abnormality occurs, the patient should discontinue use and notify the doctor immediately.
After mixing with the reconstituted solution according to the instructions, the drug can be stored at a temperature of 25 degrees Celsius. The product after reconstitution should be used within 8 hours and can be stored in the original vial or in syringe.
The drug is used at a dose of 1.3 mg / m2 / each dose, intravenously for 3-5 seconds, used twice a week and maintained for 2 weeks. Then stop the drug for 10 days. When the course of drug therapy lasts more than 8 cycles, the standard dose can be continued or maintained at once weekly for 4 weeks and discontinued for the next 13 days.
During the process of storing and preparing drugs, you need to be really careful. Patients or caregivers should use gloves and protective gear to prevent direct skin contact to avoid irritation.
Some side effects are noted as asthenia, diarrhea, nausea, loss of appetite, constipation, peripheral neuropathy, fever, vomiting, anemia, neutropenia, thrombocytopenia.
