Uses of Zenalb 20

Zenalb 20 is an intravenous solution with the main ingredient Albumin, a very important protein of the body. Zenalb 20 is used for volume resuscitation in hypovolemic emergencies where other methods have not been effective. So what is the use of Zenalb 20 and how to use it? The following article will provide the necessary information about the drug Zenalb 20 to readers.

1. Uses of Zenalb 20

Main ingredients in each 50 ml Zenalb 20 bottle:
Human Albumin 20%: 10 grams; Excipients: Ethanol, Sodium chloride, Sodium hydroxide, Sodium Acetate Trihydrate, Acetic Acid, Hydrochloric Acid, Octanoic Acid. Zenalb 20 solution is hypotonic Albumin because of its low salt content but high osmotic pressure. The drug is for intravenous use only.
The role of Albumin is very important, participating in 2 main functions of serum when helping to create and maintain 70-80% of colloidal pressure, acting as a binder and transporter of small molecules plasma as steroid hormones, fatty acids, bilirubin and even drugs in the blood.
Thanks to the effect on the colloidal osmotic pressure of plasma, infusion of Albumin into the blood helps to increase the circulating plasma volume, the rate of 1 gram of Albumin increases by 18 ml of blood volume. This extra blood volume helps reduce blood viscosity and Hematocrit. Albumin preparations do not contain clotting factors, so they do not cause blood clots or affect the blood clotting process of the body.

2. Indications and contraindications of Zenalb 20

Indications of the drug Zenalb 20
The drug Zenalb 20 is indicated in the emergency cases of shock or shock due to hypovolaemia that have failed other methods. Specifically:
Severe burns. Prevention of dehydration, electrolytes and hemoconcentration. Hypoproteinemia, especially when too much Albumin is lost in a short time. Hyperbilirubinemia in congenital hemolytic disease is common in neonates with combination therapy with transfusion transfusion. Albumin is used in combination for blood dilution in cardiopulmonary bypass surgery. Respiratory distress syndrome in adults. Contraindications of the drug Zenalb 20
Do not use the drug Zenalb 20 in the following cases:
There is a history of hypersensitivity to human Albumin or any component of the drug. Heart failure, severe anemia. Increased blood volume, pulmonary edema.

3. Dosage and how to use Zenalb 20

How to use : Zenalb 20 is a solution for intravenous infusion.
Dosage: Depends on clinical condition and patient response. Clinical status was determined based on hemodynamic monitoring including central venous pressure, pulmonary arterial blood pressure.
Infusion rate: No more than 1-2ml/min, adjusted for each specific patient. Maximum do not transmit more than 250g/48 hours.
Acute hypovolemic shock: Initial dose: 25g albumin (125ml 20% solution) in adults and 1g/kg in children. Repeat a dose after 15 - 30 minutes if necessary. Hypoproteinemia: Maximum dose 2 g/kg/day. Burns: 125ml of Zenalb 20% solution infused with solutions containing electrolytes such as Na, K, Cl,... Dosage and infusion time Albumin depends on the burn area, the required dose is enough to reduce hemoconcentration and restore plasma volume. Children with burns that do not require emergency treatment: use 6.25 - 12.5g. Hyperbilirubinemia in neonates: Intravenous dose of 1g/kg is given before transfusion is indicated. Or add Albumin dose 1.5 - 2.5g/100ml of blood transfusion. Before infusing large amounts of Albumin into the body, the patient should be incubated at room temperature or at body temperature.

4. Side effects of the drug Zenalb 20

When using Zenalb 20, patients may experience some unwanted side effects such as:
Uncommon: nausea, vomiting, allergies, urticaria, anaphylaxis. In case of clotted particles in the infusion albumin solution or the patient has hereditary anti-protein antibodies, an anaphylactic reaction may occur. When there are signs of anaphylaxis, albumin infusion should be stopped immediately, treated with antihistamines and supportive therapy.

5. Interactions of drugs Zenalb 20

Zenalb 20 may interact if used concurrently with the following drugs:
Coagulation activating factor: Stimulates the conversion of Kininogen present in the blood into Kini causing hypotension. In case of contaminated albumin solution, aluminum impurities can infect people with kidney failure, leading to bone dystrophy and brain disease.

6. Notes when using Zenalb 20

Circulatory disturbances such as overload, blood thinning or pulmonary edema may occur when large amounts of Albumin solution are infused into the body, especially in patients with low cardiac output because rapid plasma volume expansion will occur. Caution should be exercised in injured or postoperative patients because Albumin can cause hypertension, leading to bleeding in undetected sites. Concomitant infusion of electrolyte solutions is required in case the patient is dehydrated. Caution should be exercised when infusing large amounts of Albumin in patients with renal dysfunction because it can lead to metabolic alkalosis due to electrolyte imbalance. To prevent patients from becoming anemic during infusion of large amounts of Albumin, red blood cells should be replenished. Zenalb 20 solution is proven safe for pregnant women. There have been no reports of harmful effects on the neonate, so it can be used in nursing women. Absolutely do not use when detecting cloudy solution or precipitation. Above is all information about the use of Zenalb 20 medicine. If you still have questions, you can consult your doctor/pharmacist. Zenalb 20 solution is only used, manipulated and under the supervision of a doctor/medical staff to ensure patient safety.
