Application of shoulder arthroscopy in the diagnosis and treatment of adhesions (freeze) of the shoulder joint

Arthroscopic surgery in general and shoulder arthroscopy in particular are now increasingly developed and widely applied in the diagnosis and treatment of shoulder diseases. In particular, shoulder arthroscopy to treat frozen shoulder is indicated when other treatments are not effective such as taking pain relievers and anti-inflammatory drugs.

1. What is shoulder arthroscopy?

Shoulder arthroscopy is a technique that allows doctors to view the internal structures of the shoulder joint to examine, diagnose and treat diseases or injuries that cause damage to the shoulder joint including ligaments, tendons, cartilage, joint.
Initially, the patient is anesthetized to make a small cut in the skin. From this small line, a sterile solution is injected into the shoulder joint with the effect of sharpening the image during endoscopy. Then the endoscope is inserted and shows the entire inside of the shoulder joint. Based on the images of the endoscope, the doctor will check to diagnose or treat the lesions appropriately.

2. Application of shoulder arthroscopy in the diagnosis of frozen shoulder disease

The shoulder joint is a complex structure consisting of the collarbone, shoulder blade, humerus, and crest. To move, the joint is surrounded by synovial sacs, joint capsule, and muscle tendons. In diseases of the shoulder joint, ankylosing spondylitis is the stiffness and thickening of the shoulder capsule, causing pain and limitation of movement with varying degrees of severity.
If other imaging techniques such as X-rays are applied, the damage inside the joint will not be clearly visible. Therefore, shoulder arthroscopy is a modern and advanced technique that allows accurate diagnosis of damage and inflammation inside the joint, thereby helping doctors orient appropriate treatment.

3. Endoscopic treatment of shoulder ankylosing spondylitis

There are many treatment methods for ankylosing spondylitis, frozen shoulder, including medical treatment with corticosteroid injections for pain relief and anti-inflammatory, physical therapy. However, adhesive inflammation, frozen shoulder is a disease that is difficult to treat. Because in addition to pain-relieving and anti-inflammatory treatment, it is necessary to perform surgery to remove the condition of freezing, ankylosing, from which, joint function can be restored.
In the following cases, arthroscopic surgery for ankylosing spondylitis will be indicated to remove the adhesions and release the shoulder capsule:
Adhesive inflammation, prolonged frozen shoulder (more than 4 months). No response to treatment with medical methods and physical therapy (treatment time is about 3 - 6 months). The disease progresses to severe stiffness, making it impossible to move. The endoscopic technique for treatment of shoulder ankylosing spondylitis is performed as follows:
Anesthesia of the brachial nerve Injecting topical corticosteroids The doctor performs procedures such as manipulating the shoulder joint to dilate the joint capsule, helping to increase mobility. of the joint. After surgery, the patient was treated with pain relief, anti-inflammatory and rehabilitation exercises by physical therapy.

4. Advantages of shoulder arthroscopy

Compared with open surgery, shoulder arthroscopic surgery has advantages such as:
Less pain Faster recovery time Low risk of complications Low bleeding risk Reaching treatment goals

Shoulder arthroscopy has been widely used in the diagnosis and treatment of shoulder osteoarthritis, including adhesions and frozen joints. This method brings high treatment efficiency and helps patients recover joint function early.
