1. What is an episiotomy?

The vagina and perineum (the area between the vaginal opening and the anus) are very elastic. During the birth of your baby, they need to stretch to allow the baby to be born. Sometimes, during delivery, your midwife or obstetrician will need to make an episiotomy to create a larger vaginal opening. This is called an episiotomy.
The cut will be sutured by the midwife or obstetrician after delivery. Episiotomy is not done during each labor, only when needed. They are done if extra help is needed to deliver the baby or to prevent the muscles and skin from tearing. Symptoms often occur with first-time vaginal birth.

2. Reasons for episiotomy

3. Pain relief after episiotomy

Talk to your midwife or obstetrician about the options you have for dealing with any pain you experience. Although you may feel pain after an episiotomy, this is unusual for more than 2 weeks.
Pain relievers
Paracetamol tablets can really help with pain and are safe to take. The usual recommended dose is two 500 mg tablets taken every 6 hours. This is safe to take while you are breastfeeding.
Diclofenac in tablet or suppository form is also an effective pain reliever. Suppositories are inserted into your back aisle. It lasts longer and is given every 18 hours. Your midwife will show you how to use the suppository.
Aspirin is not recommended as it can be passed to your baby through breast milk.
Your obstetrician can prescribe pain medication for you while you recover in the postpartum care area.
When you get home, if paracetamol and diclofenac don't help, talk to your midwife or GP. They may need to examine you in case the area has become infected. Signs of infection:

More tips to help cope with pain
Rest and lie on your side as much as possible. Place an ice pack or ice wrapped in a towel over the wound. Do this for 20 to 30 minutes at a time a few times a day. Rest at least an hour between ice sessions. In particular, during the first 3 days, using an ice pack can really reduce swelling and inflammation around the wound. When you are sitting, using a donut-shaped cushion can help you feel more comfortable. Another trick when sitting is to press your cheeks together. This relieves some of the pressure on the wound. A warm bath can help you feel more comfortable as well as give you a chance to relax. Some women take 'Arnica'. This popular herbal remedy is said to encourage wound healing and reduce bruising. There is no convincing scientific evidence to prove that it works.

4. Sitting position after perineal incision

4.1. Urinating Clean the perineum after each use of the toilet. Pour warm water over the vaginal area after you go to the bathroom. Pouring warm water over the outside of your vagina when you pee can also help keep your urine from burning.
When you are going to the bathroom to pee, bend forward towards your knees. This helps guide urine out of the wound. Your bottom and back may feel a little bruised after the birth of your baby.
4.2. It's normal to feel worried about stitches when you have a heavy bowel movement. Don't worry, straining to go to the bathroom won't cause your stitches to fall out or the wound to open. There are things you can do to make pooping more comfortable.
Place a clean pad next to the wound and press gently as you urinate. This may relieve some of the pressure on the wound. Gently wipe your butt from front to back. This helps prevent germs from infecting the wound. Try not to get constipated. Drink a lot of water. Eat foods rich in fiber. These include vegetables, fruit, whole-grain oats, whole-wheat bread, and pasta. Taking a laxative may be helpful. Laxatives are medicines used to treat constipation. Laxatives can soften stools, making them easier to pass. 4.3. Having sex after an episiotomy There are no strict guidelines for when to start having sex again after having a baby. Some experts suggest that you should wait until the bleeding stops to reduce your risk of infection.
Do things at your own pace. It's normal to have sex a little differently after having a baby.
Remember, it's possible to get pregnant again right after giving birth. If this is not what you want, talk to your GP about birth control methods.
Most women find that having sex for the first time after an episiotomy is uncomfortable. If you try to have sex before your episiotomy or any other tear has healed, it can be very painful.
