Uses of Cadglim 2

Cadglim 2 drug is made in the form of tablets, with the main ingredient being Glimepiride. The drug is used in the treatment of type 2 diabetes.

1. What is Cadglim 2?

Cadglim 2 medicine has the active ingredient Glimepirid 2mg and other excipients. Glimepiride is a hypoglycemic sulphonamide belonging to the sulfonylurea group. Glimepiride is mainly used in the treatment of non-insulin dependent diabetes (type 2 diabetes). The drug has a stimulating effect on insulin secretion in the beta cells of the islets of Langerhans in the pancreas. Glimepiride's potency is based on the response of these cells to a glucose stimulus. In addition, Glimepiride has effects outside the pancreas.
Indications for use of Cadglim 2 :
Treatment for non-insulin dependent diabetics (type 2 diabetes) when blood sugar cannot be controlled by diet, weight loss and exercise. In cases where the effectiveness of Cadglim 2 is reduced, it can be used in combination with insulin. Contraindications to using Cadglim 2 :
Patients with hypersensitivity to Glimepiride or other ingredients in the drug; Patients with ketoacidosis, with or without coma (in which case insulin therapy is recommended); Pregnant or lactating women, children under 8 years old; Patients with severe liver or kidney failure.

2. How to use and dose of Cadglim 2

Usage: Orally, as directed by the doctor. Should be taken before breakfast with a lot of food or before the first main meal of the day.
Dosage:
Adults (18 - 65 years old): Initial dose: 1-2mg/time/day, take the drug at breakfast or lunch. In patients sensitive to hypoglycaemic drugs, a dose of 1 mg once daily should be started; Maintenance dose: Usually 1 - 4mg/time/day. The maximum dose is 8mg/day. After reaching the dose of 2mg, the patient should increase the dose at a dose interval of not more than 2mg in 1-2 weeks depending on the blood glucose response; Elderly (over 65 years old): The pharmacokinetics of the drug is similar to that of adults, so there is no need to change the dose; Children: Children 8 - 17 years old: Take the same dose as adults; Children under 8 years old: There are no data on the use of the drug in this group of people, so it is best not to use the drug. Note: With dose levels of 1mg and 3mg, patients should refer to the dosage form of Glimepiride tablets with strengths of 1mg and 3mg.
Dosage adjustment of Cadglim 2 is required in the following cases:
Because insulin sensitivity improves when diabetes is controlled, the need for Glimepiride may be reduced if treated for a period of time. To avoid hypoglycaemia, the patient will be dosed or discontinued in time; When the patient's weight or lifestyle changes; In the presence of factors that may increase sensitivity to hyperglycemia or hypoglycemia; When used in conjunction with other non-beta-cell-directed oral antidiabetic agents. Overdose: An overdose of Cadglim 2 can cause hypoglycemia. Symptoms of hypoglycemia should be treated immediately with glucose, dose adjustment along with diet. It is necessary to closely monitor the patient's health until the danger is over.
In case of severe hypoglycemia with coma, the patient should be hospitalized immediately for treatment, intravenous glucose infusion. Patients should be closely monitored for a minimum of 24-48 hours, as hypoglycemia may recur after treatment is discontinued.
Missed dose: Patients should take the dose of Cadglim 2 as soon as they remember it. If it is almost time for the next dose, the patient should skip the missed dose and just take the next dose as scheduled, no need to double the dose.

3. Side effects of Cadglim 2

Some side effects patients may experience when using Cadglim 2 include: Asthenia, headache, nausea, abdominal pain, diarrhea, increased liver enzymes, impaired liver function (rare), itching. skin, erythema, urticaria,... If severe allergic reactions persist, the patient should discontinue the drug. Usually, when experiencing side effects of drugs, patients should notify their doctor for advice on appropriate management.

4. Be careful when using Cadglim 2

Some notes patients need to remember before and while using Cadglim 2:
Sulfonylurea drugs can cause hypoglycemia. Therefore, choosing the right patient, taking the right dose, and taking the right dose is very important to avoid the risk of hypoglycemia; Patients with renal impairment may be more sensitive to the hypoglycemic effects of glimepiride. For this group of patients, it is recommended to start with 1 mg/day, increasing the dose precisely; Patients who are weak, malnourished, have impaired liver, pancreas, adrenal function... are very sensitive to the hypoglycemic effect of the drug; The hypoglycemic effect of Cadglim 2 may be difficult to discern in elderly patients who are taking beta-blockers or other sympathomimetic agents. Hypoglycemia is also common in people who do not eat enough, after drinking alcohol, after long exercise or when used with other hypoglycemic drugs. The combined use of Glimepiride with insulin or metformin may lead to an increased hypoglycemic effect; In cases of stress (trauma, infection, fever, surgery), where glycemic control may be impaired, Cadglim may need to be used in combination with insulin or even temporarily. insulin; When secondary failure occurs with glimepiride or metformin alone, it can be used in combination with insulin. If secondary failure occurs in combination with glimepiride or metformin, insulin therapy should be initiated; There are no reports of the use of Cadglim 2 in pregnant or lactating women. According to animal studies, Glimepiride should not be used in pregnant women. Therefore, patients who are planning to become pregnant should consult their doctor and switch to insulin during pregnancy. Women who are breastfeeding should not take this medicine; Cadglim 2 should be used with caution in patients who drive or operate machinery because of the possibility of hypoglycaemia or hyperglycaemia, leading to decreased flexibility and responsiveness of the patient. This risk is especially important when starting treatment or changing therapy regimens, taking drugs on an irregular basis, etc.

5. Cadglim 2 . drug interactions

Some drug interactions of Cadglim 2:
Food does not significantly change the absorption of Cadglim 2 drug, but the rate of absorption may be slightly slower; Drugs that increase the hypoglycemic effect of Glimepiride, leading to hypoglycemia include: insulin, oral antidiabetic agents, anabolic steroids and male sex hormones, ACE inhibitors, coumarin, chloramphenicol, cyclophosphamide , disomyramide, fibrate, fenfluramine, fenyramidol, fluoxetin, MAO inhibitors, azapropazone, phenylbutazone, oxyphenbutazone, probenecid, salicylates, sulfonamides, quinolones, sympathomimetics, tetracyclines, trofosfamide, tritoqualin; Drugs that reduce the hypoglycemic effect of Glimepiride, leading to hyperglycemia include: Barbiturates, acetazolamide, diazoxid, corticosteroids, diuretics, epinephrine (adrenaline), other sympathomimetic drugs, laxatives. colon, glucagon, high dose nicotinic acid, estrogen and progesterone, phenytoin, phenothiazines, thyroid hormone, rifampicin; H2-receptor antagonists, reserpine, clonidine may increase or decrease the hypoglycemic effect of Glimepiride; Under the influence of sympathomimetic drugs such as clonidine, guanethidine, beta-blockers and reserpine, signs of sympathomimetic counterregulation of hypoglycaemia may be reduced or eliminated; Heavy drinking or long-term alcohol consumption may decrease or increase the blood sugar-lowering effects of Glimepiride. During the use of Cadglim 2, the patient must still follow a strict diet, exercise and maintain an appropriate weight. At the same time, patients must not skip meals after taking the drug, follow all the instructions of the doctor to ensure the best therapeutic effect.
