Primary hypercholesterolemia, adjunctive treatment with diet to reduce elevation of total cholesterol (C-total), apolipoprotein B (Apob), low-density lipoprotein cholesterol (LDL-C), triglycerides (TG) and non-high-density lipoprotein cholesterol and to increase high-density lipoprotein cholesterol (HDL-C) in patients with primary or mixed hyperlipidemia . Homozygous Familial Hypercholesterolemia (HoFH) Vytorin is indicated for the reduction of elevated total cholesterol and LDL-C in elderly patients with HoFH. Vytorin should be used as an adjunct to other lipid-lowering treatments in these patients or if these are not available. 3. Dosage and how to use Etivas 20


Usage:
Etivas 20 should only be used by adults, not recommended for children. Patients should be on a cholesterol-lowering diet before initiating and during treatment. Patient-specific dosing is based on baseline LDL-C levels, patient response, and treatment goals. The drug should be taken once a day in the evening, with or without food. It is recommended to use a low starting dose at which the drug is effective. Dosage:
Extended dose from 1 tablet (10/10 mg)/day to 10/80 mg/day, taken once in the evening. When dosage adjustment is required, it is necessary to follow the interval of at least 4 weeks, increase to a maximum of 10/80 mg/day and take it once in the evening. The 10/80 mg dose is only recommended for use in patients at high risk for cardiovascular complications or who have not achieved treatment goals at lower doses. The recommended dose for patients with homozygous familial hypercholesterolemia is about 10/40 mg/day or 10/80 mg/day in the evening. The 10/80 mg dose should only be used when the benefits outweigh the risks. Etivas 20 should be used as an adjunct to other lipid-lowering treatments (such as LDL apheresis) in these patients or if these are not available. No dose adjustment of Etivas 20 is required in patients with moderate renal impairment. In case of severe renal impairment (creatinine clearance ≤ 30 mL/min), a dose of 10/10 mg/day should be used and close monitoring is required. Elderly: No dose adjustment of Etivas is necessary. Hepatic impairment: No dose adjustment is required in patients with mild hepatic impairment. The drug should not be used in patients with moderate or severe hepatic impairment. If combined with other drugs, the drug should be taken 2 hours before or 4 hours after taking the bile acid absorber. In combination with Etivas 20 in patients receiving Cyclosporin or Danazol, the dose should not exceed 10/10 mg/day. Do not exceed 10/20 mg/day when used in combination with patients taking Amiodarone, Verapamil or Diltiazem, the dose should not exceed 10/40 mg/day while on Amlodipine.

4. Contraindications to taking Etivas 20


Etivas 20 should not be used in the following cases:

Used in combination with Danazol, Gemfibrozil, Cyclosporin. Secondary myopathy

Gemfibrozil, Danazol, Cyclosporine.
