Glucofine 1000mg dose for patients with renal impairment Assess glomerular filtration rate (GFR) prior to initiating Glucofine 1000mg and at least annually. Accordingly, the total maximum daily dose (divided into 2-3 times/day) of Metformin according to the patient's glomerular filtration rate is as follows:
GFR = 60-89 (mL/min/1.73 m2): maximum total dose daily 2000mg (divided into 2-3 times/day); GFR = 45-59 (mL/min/1.73 m2): maximum total daily dose of 2000mg (divided into 2 - 3 times/day); GFR = 30-44 (mL/min/1.73 m2): total maximum daily dose of 1000mg (divided into 2 - 3 times/day); GFR < 30 (mL/min/1.73 m2): Contraindicated. 2.6.
