Uses of Ultomiris

1. What is Ultomiris?


Ultomiris drug has the main ingredient of ravulizumab, the content is 100mg/ml, prepared in the form of an injection solution, used by intravenous infusion.
Mechanism of Ultomiris: It works by inhibiting the complement protein C5, which is part of the body's immune system.

2. What effect does Ultomiris have?


Ultomiris is used to treat paroxysmal nocturnal hemoglobinuria (PNH) in adults and children 1 month of age and older.
It is used to treat atypical hemolytic syndrome (aHUS) in adults and children over 1 month of age. This medication is not used to treat HUS that is associated with Shiga toxin-producing E. coli bacteria.
Ultomiris is also used to treat adults with myasthenia gravis (gMG) who are positive for anti-acetylcholine receptor (AChR) antibodies.

3. How to use Ultomiris


Ultomiris is given intravenously. Usually given over about 2 hours in adults and up to 4 hours in children. If an adult has PNH or aHUS, first and second doses are given 2 weeks apart, then infusion every 8 weeks.
Children 1 month of age and older with aHUS, take the dose as prescribed by the doctor, can take the first dose and the second dose 2 weeks apart, then every 4 weeks or every 8 weeks, depending on on the child's weight. If you are changing your treatment from Soliris to Ultomiris, the starting dose of Ultomiris should be taken 2 weeks after your last dose of Soliris. After each infusion of Ultomiris, it is recommended to monitor for at least 1 hour for allergic reactions.

4. Dosage of Ultomiris

Usual dose for adults and children with paroxysmal nocturnal hemoglobinuria:
5kg to less than 10kg: Loading dose: 600 mg IV
Maintenance dose: 300 mg IV every 4 weeks starting 2 weeks later First loading dose
10kg to less than 20kg: Loading dose: 600 mg IV
Maintenance dose: 600 mg IV every 4 weeks starting 2 weeks after first loading dose
20kg to less than 30kg: Loading dose: 900 mg IV
Maintenance dose: 2100 mg IV every 8 weeks, starting 2 weeks after the first loading dose.
30kg to less than 40kg: Loading dose: 1200 mg IV
Maintenance dose: 2700 mg IV every 8 weeks, starting 2 weeks after loading dose
40kg to less than 60kg Loading dose: 2400 mg IV
Maintenance dose: 3000 mg IV every 8 weeks starting 2 weeks after loading dose
60kg to less than 100kg: Loading dose: 2700 mg IV
Maintenance dose: 3300 mg IV every 8 weeks, starting 2 weeks after loading dose
Regular dose Usual for adults and children with atypical hemolytic syndrome:
5 kg to less than 10 kg: Loading dose: 600 mg IV

10 kg to less than 20 kg: Loading dose: 600 mg IV

40 kg to less than 60 kg: Loading dose: 2400 mg IV
Maintenance dose: 3000 mg IV every 8 weeks starting 2 weeks after loading dose
60 kg to less than 100 kg: Loading dose: 2700 mg IV
Maintenance dose: 3300 mg IV every 8 weeks, starting 2 weeks after loading dose
100 kg or more: Loading dose: 3000 mg IV
Maintenance dose: 3600 mg IV every 8 weeks, starting at 2 weeks after the loading dose.
In case of missed dose or overdose
Call your doctor for instructions if you miss a dose of Ultomiris injection. In a hospital or medical facility, an overdose is treated quickly.

5. Ultomiris side effects


Ultomiris can cause serious side effects including infusion-related reactions. Symptoms of an infusion-related reaction may include lower back pain, pain with the infusion, and feeling weak or uncomfortable in the arms or legs. More severe symptoms may include chest pain, difficulty breathing, swelling of the face, tongue, or throat, feeling weak or unconscious.

7. Notes when taking Ultomiris


Ultomiris is a medicine that affects the immune system, which can decrease the immune system's ability to fight infections.
Ultomiris increases your risk of getting a serious and life-threatening meningococcal infection. Meningococcal infection can be fatal if not recognized and treated early.
You must get the meningococcal vaccine at least 2 weeks before your first dose of Ultomiris if you have not already received this vaccine.
Ultomiris may also increase your risk of other serious infections. Make sure your child is vaccinated against Streptococcus pneumoniae and Haemophilis influenzae type b (Hib) if being treated with Ultomiris. Call your doctor right away if there are any new signs of infection.
Some people may have an increased risk of getting gonorrhea (sexually transmitted disease) while using this medicine. Talk to your doctor about safe ways to avoid getting an infection during sex. Tell your doctor right away if you experience: pain or burning when urinating, pain or redness in the genital or rectal area, unusual vaginal bleeding; unusual discharge from the penis or vagina.
