1.1. What is Vertisum Tablets? Vertisum Tablets belongs to the group of antipsychotics. Vertisum Tablets includes the following ingredients:
● Active ingredient: Prochlorperazine maleate BP 5mg. Excipients: Lactose monohydrate (Pharmatose 200M), microcrystalline cellulose (PH 101), corn starch, sodium crosscarmellose, sodium sulphate, microcrystalline cellulose (PH 102), magnesium stearate, anhydrous colloidal silica.
The drug is prepared in the form of 10 tablets, 1 blister, box of 10 blisters.
1.2. What are the uses of Vertisum Tablets? Prochlorperazine maleate: is a phenothiazine, with antidepressant activity on the central nervous system.
Prochlorperazine has sedative properties, fast drug tolerance. The drug has antiemetic, antipruritic, weak antihistamine effect, inhibits the thermoregulatory center, relaxes smooth muscle, so it can be used in local anesthesia.
Prochlorperazine is well absorbed from the gastrointestinal tract, metabolized mainly in the liver, the drug is excreted in urine and bile. Blood concentrations of the drug when administered intramuscularly are usually higher than when administered orally.
Vertisum Tablets is used in the following cases
Dizziness due to Meniere's syndrome , dizziness due to labyrinthitis Cases of nausea and vomiting associated with migraine headaches of any etiology. Indications for the treatment of schizophrenia (especially the chronic phase). Indicated to support the treatment of symptoms of restlessness and anxiety.

2. Usage of Vertisum Tablets


Vertisum Tablets are made in the form of tablets that should be taken orally.
Dosage of Vertisum Tablets can be referred to as follows:
Dizziness due to Meniere's syndrome: dose 5mg/time x 3 times/day, increase dose as needed to total dose of 30mg/day. The dose can then be gradually reduced to 5-10 mg/day. Prevention of nausea and vomiting: dose 5 - 10mg/time, can be used 2 - 3 times/day. Treatment of nausea and vomiting: dose 20mg as soon as symptoms of vomiting, nausea, if still symptoms can take another 10mg 2 hours later. Schizophrenia and other psychotic disorders: doses of 75-100 mg/day are considered effective. Initially 12.5 mg twice daily for 7 days, the daily dose may then be increased by 12.5 mg at intervals of 4-7 days until a response is achieved. After several weeks of taking an effective dose, try to reduce the dose. Sometimes total daily doses of 50 mg or even 25 mg have been shown to be effective. Children used to prevent and treat vomiting and nausea: if possible, avoid using prochlorperazine in children, when necessary, the recommended dose is 0.25 mg/kg body weight, 2-3 times a day. Prochlorperazine is not recommended for children under 10 kg or under 1 year of age. Treatment of overdose:
Symptoms: drowsiness, unconsciousness, hypotension, tachycardia, ECG changes, ventricular disturbances, hypothermia, extrapyramidal disorders.
Treatment: If the patient is detected early (up to 6 hours), it can be detoxified by gastric lavage, in addition, activated charcoal can be used. Currently, there is no specific drug to treat overdose, mainly symptomatic treatment.

3. Contraindications of Vertisum Tablets


Vertisum Tablets are contraindicated in patients with known hypersensitivity to prochlorperazine or any of its ingredients.

4. Warnings and Precautions for Vertisum Tablets


Avoid using this drug in patients with liver failure, renal failure, Parkinson's disease, hypothyroidism, heart failure, pheochromocytoma, myasthenia gravis, prostatic hypertrophy, patients sensitive to phenothiazines, history of glaucoma narrow angle pressure or agranulocytosis.
Close monitoring is required in patients with epilepsy or a history of seizures, as phenothiazines may lower the seizure threshold.
Regular monitoring of CBC is recommended because agranulocytosis has been reported. Fever or unexplained inflammation may be a sign of a hematopoietic disorder and a complete blood count should be checked.
If fever is unexplained, treatment should be discontinued immediately, as it may be a sign of a neuroleptic malignant syndrome (pallor, hyperthermia, autonomic nervous system dysfunction, altered cognitive , muscle spasticity).
Nausea, vomiting, and insomnia rarely occur after a patient abruptly stops taking high doses of sedatives. Recurrence as well as extrapyramidal disturbances may occur after discontinuation of the drug, so a gradual dose reduction is recommended.
Phenothiazines can cause severe prolongation of the QT interval, increasing the risk of ventricular arrhythmias and the risk of torsades de pointes potentially leading to sudden death. QT prolongation is more likely to be exacerbated by bradycardia, hypokalemia, or congenital or acquired QT prolongation (eg, drug-induced), so the benefits and risks need to be assessed prior to treatment. start using this medicine.

5. Precautions when using Vertisum Tablets

Neonates exposed to antipsychotic drugs during the third trimester of pregnancy are at risk for adverse events such as extrapyramidal disorders or symptoms such as agitation, increased or decreased muscle tone, tremors, drowsiness, poor breathing, poor appetite. Breastfeeding is not recommended during treatment as this drug may be excreted in breast milk.

6. Side effects of the drug Vertisum Tablets


Some side effects may be encountered when taking the drug such as:
Drowsiness, dizziness, blurred vision, skin reactions, low blood pressure. Neuroleptic malignant syndrome (NMS). Cholestatic jaundice: when there are symptoms such as fever, jaundice, liver function should be rechecked, drug should be discontinued if tests show abnormalities. Leukopenia, granulocytopenia: sudden sore throat and signs of infection may appear. If the white blood cell count falls, treatment should be discontinued, and the initiation of antibiotics and other appropriate treatments considered. ECG changes: usually nonspecific, reversible Q and T wave distortions. Although phenothiazines do not make the patient dependent, stopping the drug suddenly after taking it for a long time can cause temporary symptoms such as nausea and vomiting, dizziness, and tremors.
