This is a complex congenital heart disease, with cyanosis due to oxygen-poor blood from the veins returning to the right ventricle, through the ventricular septal defect to the left ventricle and then through the aorta into the great circulation. Anatomically, there are 4 types:
Body 1; Pulmonary artery trunk and 2 normal pulmonary artery branches, blood from the aorta through the ductus arteriosus into the pulmonary artery. Type 2: no trunk of the pulmonary artery but 2 branches of the pulmonary artery and normal aorta, blood from the aorta through the aorta into 2 branches of the pulmonary artery. Type 3: no pulmonary trunk, atrophy of the pulmonary branches, atrophy of the aorta, blood to the lungs mostly through the collateral arteries. Type 4: no pulmonary trunk, 2 branches of the pulmonary artery and atrophy of the aorta, blood to the lungs completely through the collateral arteries. Prognosis and indications for treatment depend on the type. For types 1 and 2, the prognosis is good because it can be completely corrected by inserting a prosthetic valve in the atrophied pulmonary valve, and closing the ventricular septal defect (usually through 2 stages, can be operated as early as 12 months, the prognosis is poor). better quality). For types 3 and 4, the prognosis is poor because it cannot be completely repaired, but can only be temporarily corrected to provide a partial blood supply to the lungs: create a Glenn or Fontan shunt.
