HCG begins to be born as soon as the egg is fertilized and begins to implant on the lining of the uterus. At this time, the concentration of HCG will appear in the blood, rapidly increasing until reaching the maximum concentration at two and a half months. After this period, the amount of HCG will gradually decrease to a stable level around the fourth month and continue until delivery.
A quantitative blood test can detect HCG at a concentration of 2mIU/ml for about 8 days after ovulation. In a normal pregnancy, HCG levels increase by at least 66% within the first 48 hours and 100% within 72 hours. If the amount of HCG rises below the above level, it is predictive of miscarriage or ectopic pregnancy.
The concentration of HCG increases from the day of egg implantation and peaks at day 60-70 of gestational age, then gradually decreases to a lowest level at day 100-130. If the HCG level is too high, above 100,000mIU, pregnancy should be considered.
Combined with ultrasound, quantitative HCG tests can identify early pregnancy loss and support the diagnosis of ectopic pregnancy. If the beta HCG level is above 1700 to 2000 mIU/ml,(1.7->2 IU ) a transvaginal ultrasound will show a gestational sac in the uterine cavity.
