Rapid intervention in cerebral vessels saves patients with life- threatening acute stroke.

Anticipating the increasing and rejuvenating trend of stroke, Vinmec Da Nang International General Hospital has built a highly professional stroke team, implementing intervention procedures according to European standards. Thanks to that, the stroke team can always perform quick intervention with the most appropriate methods when a patient arrives, in many cases within a few minutes of admission. Since then, the patient has recovered spectacularly, without sequelae despite the previous very poor prognosis.

Only 20 minutes to complete cerebral occlusion recanalization

On the afternoon of November 7, 2022, Mr. Dang Truong Son (45 years old) suddenly collapsed, lost consciousness, and coma. Mr. Son was diagnosed with an acute stroke and first aid at a hospital in Da Nang and transferred to Vinmec International General Hospital. Upon receiving information that the patient was on the way, Vinmec Da Nang immediately activated and mobilized the stroke team to be ready to receive and process the case. Therefore, as soon as he arrived at Vinmec, Mr. Son was taken straight to the Cerebral Vascular Intervention Room. With a state-of-the-art DSA digital subtraction angiography machine modern interventional tools and the team's professional skills, after only 20 minutes, 2 large blood clots in the basilar artery were removed.

From a complete artery blockage that led to coma and a very high risk of death, after the thrombectomy, the brain was quickly revascularized, and Mr. Sơn regained full consciousness. Remarkably, his limbs regained movement just 24 hours later, and he was able to walk on his own. "After 6 days of intervention, I returned to my normal state as if I had never had a stroke. It's hard to believe, but I am truly ready to return to work now," Mr. Sơn shared on the day of his discharge.

In another recent spectacular case, Mr. Vo Thanh Loc (born in 1977, in Son Tra District, Da Nang) also came back from the dead after timely intervention to remove blood clots with mechanical devices. When he became lethargic, Mr. Loc was examined and had a CTA scan, diagnosed with an acute stroke due to occlusion of the left middle cerebral artery, a severe condition. Symptoms included inability to speak, and paralysis of the right face and left side of the body. Due to the blockage of a large blood vessel after using thrombolytic drugs that did not dissolve the blood clot, meaning the blood vessels could not be reopened, the hospital he examined and diagnosed advised to transfer him to Vinmec Hospital for further treatment.

Just 5 minutes after being admitted to the hospital, doctors removed the blood clot and revascularized Mr. Loc's cerebral circuit. Thanks to that, from being in a coma, on ventilator support, with a very high risk of death, after the blood clot was removed, Mr. Loc regained consciousness after 16 hours, and the strength of his paralyzed limbs improved significantly. After being in the Intensive Care Unit for 24 hours for monitoring and care, the patient was transferred to the stroke department for standard treatment and early rehabilitation. After 2 weeks of intensive treatment, Mr. Loc is healthy, eating well, walking and living normally, and his language is gradually recovering.

Stroke is becoming increasingly common among younger individuals. Who should be alert?"

The effectiveness of stroke intervention methods and good recovery of the above severe stroke cases is a positive signal that helps us to be more optimistic when unfortunately having a relative with a stroke. The more successful stroke intervention is, the more doctors "appreciate" the value of every moment for the life and survival of the patient.

The success in intervening in many stroke cases at Vinmec Da Nang comes from the process of the hospital implementing the most advanced and optimal stroke emergency procedure, meeting the criteria for emergency stroke treatment of the European (ANGLES). And especially the cerebrovascular intervention team is especially professional in the emergency treatment of cerebral reperfusion for stroke patients.

Having modern diagnostic imaging equipment such as a 640-slice CT scanner, and a high-resolution 3.0 Tesla MRI scanner, and mastering many new techniques has helped doctors to choose the optimal method for each case. Therefore, as a Comprehensive Stroke Center to lead the Central and Central Highlands regions in terms of technology and quality, doctors have saved lives and limited deaths for many stroke patients as well as cerebral vascular malformations that pose a risk of causing acute stroke.

Stroke is increasingly becoming common among younger, so MSc. Dr. Ton That Tri Dung - Head of the Department of Examination and Internal Medicine, Vinmec Da Nang International General Hospital, who directly performs cerebral vascular intervention, recommends that people at risk such as those with cardiovascular disease, diabetes, etc. need to strictly control their disease. In Mr. Son's case, the blood clots may have moved from the heart because 3 days before, the patient had stopped taking anticoagulants on his own. Mr. Loc has a history of high blood pressure but has not taken medication. Even cerebral hemorrhage can occur in healthy young people with few or even no risk factors. Therefore, Dr. Dung recommends not to be subjective when having symptoms such as sudden headache, and vomiting and should be hospitalized early. When headaches persist, recur, or are of unknown cause, it is necessary to screen for the cause by MRA or CTA to detect early cerebral aneurysms, cerebral vascular malformations, atherosclerotic stenosis of cerebral arteries... to have a treatment plan and improve, preventing stroke from occurring.
