2.1. How to use A cnotren
A-Cnotren is used orally: Take the medicine with a glass of warm water to increase the absorption of the medicine, helping the treatment process achieve the best results. The medicine should be taken during meals and should not be taken with alcohol, beer, etc.
2.2. Dosage of A cnotren
Dosage for adults and children over 12 years old:
- Starting dose: 0.5 - 1 mg/kg/day, divided into 2 times for 15 - 20 weeks. Maximum: 2 mg/kg/day for very severe acne, acne on the chest or back.
- Maintenance dose: 0.1 - 1 mg/kg/day. Treatment should be stopped for 2 months before re-using.
- In severe refractory cystic acne, the dose may be increased to a maximum of 2 mg isotretinoin/kg body weight per day.
- For the treatment of keratinization disorders, the dose of isotretinoin may be increased to 4 mg isotretinoin/kg body weight per day.
Dosage for children under 12 years of age: Not for use in children under 12 years of age.
In patients with severe renal impairment, treatment should be initiated at a lower dose (e.g. 10 mg/day). The dose should then be increased to 1 mg/kg/day or until the patient receives the maximum tolerated dose.
Intolerant patients:
In patients who exhibit severe intolerance to the recommended dose, treatment may be continued at a lower dose with the consequent longer treatment duration and increased risk of relapse. To achieve the maximum possible effect in these patients, the dose should be continued at the highest tolerated dose.
Treatment for missed doses:
If the patient forgets a dose, the dose must be taken immediately. Note that the dose must not be doubled when the next dose is missed.
Treatment for overdose:
If the patient uses an overdose, the drug must be stopped immediately and the doctor must be informed to monitor the patient's condition. Some mild symptoms may appear and subside without treatment such as: nausea, vomiting, drowsiness, itching.

Do not use the drug in the following cases:

- Patients with hyperlipidemia.

- Alcohol can increase the risk of increased triglyceride levels, increased side effects and toxicity of isotretinoin.
- Isotretinoin can reduce serum Progesterone levels, leading to reduced contraceptive effect in patients using drugs containing progesterone.
- In addition, caution should be exercised when using A Cnotren with the following drugs: Doxycycline, Tetracycline, verteporfin, topical acne medications.

5. Side effects of A cnotren
Common:
- On the skin and mucous membranes:
- Frequent cheilitis

- Dry mucous membranes of the mouth, vagina, throat and nose, sometimes accompanied by nosebleeds and hoarseness.

Uncommon:
- Cases of rash, itching, erythema nodosum and vasculitis, increased eye pigmentation.
- Melasma on the face.

- Increased sensitivity to sunlight.
- Hair loss or changes in degree (smaller), usually reversible after stopping the drug.
- Irritation of the urethra, blood in urine.
- Changes in the skin microflora, risk of local or systemic infection, especially due to Staphylococcus aureus.
- Staphylococcal granulomatosis in periungual lesions or lesions covered by acne.
- Nail dystrophy (foot, hand).
Rare:
- The skin becomes very fragile, which can lead to peeling, especially on the face after hair removal.
- Hypertrophic scars after dermabrasion.
- Cerebral edema causing nausea, vomiting, headache, visual changes.
Special cases:
Rare acne flares: Follicular lesions rapidly develop into inflammatory, purulent lesions, then ulceration - necrosis, often with systemic signs (fever, arthralgia, myalgia), and finally scarring. Treatment should be discontinued and systemic corticosteroids initiated.
