Antibiotics are commonly used medications for treating infections in different parts of the body. Therefore, identifying the infection site helps doctors choose the appropriate antibiotic for the patient. In some cases, broad-spectrum antibiotics are selected to treat infections with an unclear bacterial cause or suspected involvement of multiple bacterial groups.
1. What is Broad-Spectrum Antibiotics?
Antibiotics are chemical substances derived from microorganisms or synthesized compounds that, in small doses, inhibit or kill pathogenic microorganisms. They can be used topically or systemically with minimal or no toxicity to the body. There are 2 ways to classify antibiotics groups which are based on their spectrum of activity and chemical structure.
Each antibiotic has a specific mechanism of action that either kills bacteria (bactericidal) or inhibits their growth (bacteriostatic), targeting only certain bacterial strains, which determines its spectrum of activity. Based on this, scientists have categorized antibiotics into two groups:
- Narrow-spectrum antibiotics: These antibiotics selectively target only one or a few specific bacterial species.
- Broad-spectrum antibiotics: These antibiotics have a wide range of activity against both Gram-negative and Gram-positive bacteria.

2. Common Broad-Spectrum Antibiotic Groups
2.1. Beta-Lactam Group
Research on the chemical structure modifications of penicillins and cephalosporins within the beta-lactam antibiotic class has led to the development of carbapenems, which are potent bactericidal agents with broad-spectrum activity, particularly against Gram-negative bacteria. Some representative antibiotics in this group include imipenem, meropenem, doripenem, and ertapenem, which exhibit broad activity against both aerobic and anaerobic bacteria.
2.2. Phenicol Group
Notable antibiotics in the phenicol group include chloramphenicol and thiamphenicol, where chloramphenicol is a naturally derived antibiotic, and thiamphenicol is synthetically produced. Phenicol antibiotics have a broad spectrum of activity, covering Gram-positive cocci, certain Gram-negative bacteria, anaerobes, and atypical bacteria. However, in clinical practice, their use has declined due to widespread bacterial resistance and potential severe toxicity, particularly affecting hematopoiesis.
2.3. Cycline Group
Antibiotics belonging to the cycline group have a broad antibacterial spectrum against many bacterial strains, including atypical bacteria such as Mycoplasma pneumoniae and Chlamydia spp. Additionally, the drug is also effective against certain spirochetes such as Treponema pallidum (syphilis) and Borrelia recurrentis. Since it has been used in treatment for a long time, the rate of bacterial resistance to tetracycline is now very high. Therefore, some later cyclines such as doxycycline and minocycline can act on bacteria that have developed resistance to tetracycline. Broad-spectrum antibiotics are antibiotics that have extensive effects on many types of bacteria. Therefore, to choose the appropriate antibiotic and avoid antibiotic misuse, improper use of antibiotics, patients should select medical facilities for examination, treatment, and proper prescription.
