- Systemic candidiasis: Day 1: 3 capsules once daily; thereafter, 1 capsule once daily. Treatment lasts 4 weeks, plus an additional 2 weeks after symptoms resolve.
- Cryptococcal meningitis: Day 1: 3 capsules once daily; thereafter, 1 capsule once daily for at least 6–8 weeks until cerebrospinal fluid cultures test negative.
Dosage for neonates
- 0–2 weeks old: 3–6 mg/kg per dose, every 72 hours.
- 2–4 weeks old: 3–6 mg/kg per dose, every 48 hours.
Dosage for children
- Prophylaxis: 3 mg/kg/day for superficial fungal infections and 6–12 mg/kg/day for systemic fungal infections.
- Treatment: 6 mg/kg/day. In persistent cases, the dose may be increased to 12 mg/kg/day, divided into two doses. The maximum daily dose should not exceed 600 mg.
