1. Uses of Omez 20mg

1.1 What is Omez 20mg?

Omez contains the active ingredient omeprazole and is manufactured by Dr. Reddy's Laboratories. It is indicated for the short-term treatment of: Gastric-duodenal ulcers, gastric ulcers, reflux esophagitis, control of Zollinger-Ellison syndrome
Excipients: Mannitol; Lactose; Sodium lauryl sulfate; Anhydrous disodium hydrogen phosphate; Sucrose; Hypromellose; Methacrylic acid copolymer Type-C (Eudragit L100-55); Sodium hydroxide; Polyethylene glycol; Titanium dioxide; Purified talc.
Omez comes in the form of hard capsules, provided in a box containing 2 blisters of 10 capsules each.

Omez 20mg, with omeprazole as its main ingredient, is effective for the following conditions:

  • Short-term treatment of duodenal ulcers and gastric ulcers
  • Reflux esophagitis
  • Control of Zollinger-Ellison syndrome

2. How to Use Omez 20mg

2.1. How to Use Omez 20mg

Patients should read the instructions carefully before starting to use the medication and each time they use it again. 
The usage and administration methods may vary depending on the form of the medication, including oral, injectable, external, or suppository.
Omez 20mg capsules should be taken orally, and the dosage may be divided throughout the day based on the patient's condition. It is crucial to follow the prescribed dosage; increasing the dose or frequency does not lead to faster improvement and may increase the risk of side effects.

Usage Guidelines:

  • Take the drug at any time of day.
  • Can be taken before or after meals.
  • Swallow the capsule whole with water; do not chew or crush it.

2.2. Dosage of Omez 20mg

For Adults:

  • Duodenal Ulcers: 20 mg daily for 4 weeks, with an additional 4 weeks if necessary.
  • Gastric Ulcers: 40 mg daily for 4 weeks.
  • Reflux Esophagitis (GERD) without esophagitis: 20 mg daily for 4 weeks. Reflux Esophagitis (GERD) with esophagitis: 20 mg daily for 4 to 8 weeks.
  • Zollinger-Ellison Syndrome: Starting dose of 60 mg; may vary up to 120 mg daily. 80 mg/day can be given in 2 divided doses.

For Children: The safety and efficacy of omeprazole capsules in children have not been established.

For Elderly Patients: No dosage adjustment is necessary.

For Patients with Hepatic Impairment: Dosage reduction may be required, as omeprazole is extensively metabolized in the liver, and its elimination rate may be prolonged.
For Patients with Renal Failure: No dosage adjustment is required.

Handling Missed and Overdose Doses
If you miss a dose, take it as soon as possible. If it's close to your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose.

Overdose: 
Information on accidental overdose of omeprazole is limited. 
There is no specific antidote for omeprazole overdose. Omeprazole is tightly bound to proteins and is therefore not readily dialysable. Treatment typically involves symptomatic and supportive care. Reports of overdose ranging from 320 to 900 mg are rare. Common side effects at this dosage may include confusion, dizziness, blurred vision, rapid heartbeat, nausea, sweating, flushing, headache, and dry mouth

3. Contraindications of Omez 20mg

Omez 20mg is contraindicated in the following cases:

  • Patients who are hypersensitive to omeprazole or any of the excipients in the drug.
  • Pregnant and lactating women.
  • Driving or operating machinery ability: Use with caution, as there have been reports of side effects like drowsiness and dizziness. Those experiencing these effects should be particularly careful.
  • Pregnancy: The safety and effectiveness of omeprazole in pregnant women have not been established; therefore, it should not be used during pregnancy.
  • Breastfeeding: Omeprazole is not recommended for lactating women, as its safety and effectiveness during breastfeeding have not been established.

4. Notes on Using Omez 20mg

Use caution when administering this medication to:

  • Patients experiencing gastric issues.
  • Patients presenting any of the following symptoms: unintentional weight loss, vomiting, vomiting blood, or tarry stool.
  • Patients with a possibility of stomach cancer, as the drug may mask symptoms and lead to delayed diagnosis.
  • Individuals with liver disease, as well as pregnant or breastfeeding women.
  • Long-term use of omeprazole should be approached with caution because it can lead to gastritis.
  • Caution is also advised for children under 18 years of age, as the safety and effectiveness of the drug in this population have not been established. It is not recommended for children.
  • Patients with rare hereditary conditions such as fructose intolerance, glucose-galactose malabsorption, or sucrase-isomaltase deficiency should avoid omeprazole.
  • Treatment with proton pump inhibitors like omeprazole may increase the risk of gastrointestinal infections, such as those caused by Salmonella and Campylobacter.
  • Avoid administering esomeprazole with atazanavir, as they can interact.
  • Esomeprazole may reduce the absorption of vitamin B12. Therefore, exercise caution when using the drug in patients with low vitamin B12 stores or risk factors for impaired vitamin B12 absorption, especially during long-term treatment.
  • Concurrent use of esomeprazole and clopidogrel is not recommended.
  • Patients with musculoskeletal disorders should follow clinical guidelines and ensure adequate supplementation of vitamin D and calcium.

5. Side Effects of Omez 20mg

  • Systemic: Allergic reactions such as hypersensitivity, fever, pain, fatigue, or abdominal discomfort.
  • Cardiovascular: Headaches, tachycardia, bradycardia, palpitations, increased blood pressure, and peripheral edema.
  • Gastrointestinal: Symptoms like dry mouth, flatulence, anorexia, irritability, and discolored stools. Reports of small linear gastric fundi have occurred, though these are benign and resolve once the drug is stopped. Long-term use in patients with Zollinger-Ellison syndrome may lead to gastric and duodenal cancerous lymph nodes, believed to be a primary manifestation of malignancy.
  • Liver: Conditions such as jaundice, cholestasis, hepatitis, cirrhosis, liver failure, and hepatic encephalopathy.
  • Metabolic: Issues like hyponatremia, hypoglycemia, and weight gain.
  • Musculoskeletal: Muscle cramps, myalgia, weakness, and leg pain.
  • Nervous System and psychiatric: Symptoms may include depression, agitation, hallucinations, confusion, insomnia, restlessness, tremors, anxiety, nightmares, dizziness, paresthesia, and hemifacial dysesthesia.
  • Respiratory: Epistaxis and esophageal pain.
  • Skin: Reactions like rash, drug-induced epidermal necrolysis, Stevens-Johnson syndrome, dermatitis, urticaria, angioedema, leukoplakia, alopecia, dry skin, and hyperhidrosis.
  • Urinary: Interstitial nephritis (sometimes recurs with re-use of the medication), urinary tract infection, pus in urine, polyuria, increased plasma creatinine, proteinuria, hematuria, glucose in urine, testicular pain, gynecomastia
  • Bleeding: Rare cases of blood disorders such as thrombocytopenia, agranulocytosis, neutropenia, anemia, leukocytosis, and hemolytic anemia have been reported as well.

6. How to Store Omez 20mg
