Uses of Tempovate cream

Tempovate cream is a topical medication containing Clobetasol propionate as its main active ingredient. It is used for short-term treatment of dermatological conditions. What should be noted before using Tempovate cream?  

1. Effects of Tempovate cream  

Clobetasol propionate is a highly potent corticosteroid that is minimally absorbed through the skin. However, when applied topically, a certain amount of the drug can be absorbed into the body, making it effective for treating allergic dermatitis. It is important to note that drug absorption significantly increases when the skin is damaged, so it should be used with caution on open wounds.  

Clobetasol belongs to the glucocorticoid class and is used topically for its anti-inflammatory properties. It inhibits the synthesis of chemical mediators responsible for inflammation and suppresses the release of mediators that trigger allergic reactions, such as histamine, serotonin, and bradykinin.  

2. Indications and contraindications of Tempovate cream  

Tempovate cream is indicated for short-term treatment of redness, itching, scaling, peeling skin, eczema, psoriasis, dermatitis, and other skin discomforts. Additionally, it is used to treat eczema and dermatitis, including allergic eczema, otitis externa, photodermatitis, lichen simplex chronicus, seborrheic dermatitis, and insect bites.  

Some contraindications of Tempovate cream include:  

  • Common acne, rosacea, and perioral dermatitis.  
  • Itching around the anus and genital area.  
  • Viral skin infections such as herpes simplex and chickenpox.  
  • Patients with a history of hypersensitivity to any component of the drug.  
  • Clobetasol propionate for topical use is not indicated for primary skin infections caused by fungi or bacteria, nor for dermatological conditions in children under one year old, such as diaper rash or inflammatory dermatitis.  

3. How to use Tempovate cream  

Apply a thin layer of the cream to the affected area once or twice daily. Discontinue use if any unusual signs appear.  

Do not use Clobetasol propionate continuously for more than four weeks without medical supervision. If severe flare-ups occur, short-term treatment cycles may be repeated. For long-term corticosteroid treatment, milder corticosteroids should be considered.  

For hard-to-treat lesions, such as thickened skin areas, the anti-inflammatory effect of Clobetasol propionate can be enhanced by covering the treated area with a polyethylene film overnight to achieve the desired response.  

4. Undesirable side effects of Tempovate cream  

When used in large amounts over extended periods or on a large skin surface, the drug may be absorbed in sufficient quantities to cause symptoms of adrenal hyperfunction, similar to other topical corticosteroids. This risk is higher in infants and children, particularly when using occlusive dressings.  

When adhering to the recommended dose of less than 50g per week in adults, hypothalamic-pituitary-adrenal axis suppression is usually transient and quickly reversible after short-term treatment discontinuation.  

Additionally, prolonged use of high-dose corticosteroids may lead to localized skin atrophy, such as thinning of the skin, skin fissures, and dilation of superficial blood vessels.
