Ulsepan 40mg belongs to the group of medications for treating gastrointestinal diseases, commonly prescribed for gastroesophageal reflux disease (GERD), gastrointestinal ulcers, gastritis, prevention of ulcers caused by non-steroidal anti-inflammatory drugs (NSAID), Zollinger-Ellison syndrome.
1. How does Ulsepan 40mg work?
Ulsepan 40mg with its main ingredient Pantoprazole is a proton pump inhibitor with a structure and effect similar to Omeprazole. It is a substituted Benzimidazole. Pantoprazole is protonated into an active form in the acid secreting ducts of the gastric wall cells, where it inhibits the enzyme H+/K+ATPase, also known as the proton pump, the final stage of gastric acid secretion.
After taking Ulsepan 40mg, the antisecretory effect of Pantoprazole lasts more than 24 hours and inhibits up to 50% of gastric acid secretion after 2.5 hours of intake in healthy people. Gastric acid secretion returns to normal within 1 week after discontinuing Pantoprazole, with no rebound secretion. Additionally, Pantoprazole can also inhibit Helicobacter pylori bacteria in patients with gastric and duodenal ulcers and/or reflux esophagitis infected with this bacterium. The combination of Pantoprazole with antibiotics (Clarithromycin, Amoxicillin) can eradicate H. pylori along with healing ulcers and providing long-term disease remission.
Ulsepan 40mg is commonly indicated in the following cases:
- Reflux esophagitis in adults and children over 12 years old.
- Eradication of H. pylori in combination with other antibiotics in patients with gastric and duodenal ulcers caused by H. pylori.
- Gastric and duodenal ulcers
- Zollinger-Ellison syndrome and other conditions causing increased gastric acid secretion.
Ulsepan 40mg is contraindicated in cases of hypersensitivity to any component of the medication.
2. Dosage of Ulsepan 40mg:
Ulsepan 40mg is typically taken orally once daily in the morning, 30-60 minutes before eating. Depending on the patient and treatment goals, the dosage of Ulsepan 40mg will vary, specifically as follows:
- Gastroesophageal reflux disease: Use a dose of 40mg once daily for a short period (up to 8 weeks); however, an additional 8 weeks may be considered for patients who haven't healed after the initial treatment course.
- Gastric and duodenal ulcers: Take 40mg daily for 2-4 weeks for duodenal ulcers and 4-8 weeks for benign gastric ulcers.
- For H. pylori eradication, Pantoprazole can be combined with 2 antibiotics in a 3-drug or 4-drug treatment regimen for one week: Pantoprazole 40 mg, twice daily combined with Clarithromycin 250mg twice daily and Metronidazole 400mg, twice daily.
- Prevention of gastrointestinal ulcers due to non-steroidal anti-inflammatory drugs: Use a dose of 20mg/day orally.
- Pathological hypersecretion in Zollinger-Ellison syndrome: Use a dose of 40mg x 2 times/day, maximum 240 mg/day.
- Data on medication use in children is limited; do not use Pantoprazole for children under 12 years old.
3. Side effects of Ulsepan 40mg
Some patients using Ulsepan 40mg may experience side effects such as:
- Lymphatic system disorders.
- Anaphylactic reactions and anaphylactic shock.
- Hyperlipidemia, changes in body weight.
- Hyponatremia, hypomagnesemia.
- Sleep disorders, depression, disorientation, hallucinations, confusion.
- Headache, dizziness.
- Visual disturbances/blurred vision.
- Diarrhea, nausea, abdominal pain and bloating.
- Liver cell damage, jaundice, liver cell failure.
- Urticaria, edema.
- Stevens-Johnson syndrome, Lyell syndrome, erythema multiforme, photosensitivity.
- Hip, wrist, or spine fractures.
- Joint pain, muscle pain.
- Interstitial nephritis.
- Gynecomastia.
- Asthenia, fatigue, discomfort.
4. Precautions when using Ulsepan 40mg:
Some general notes when using Ulsepan 40mg include:
- Before using Pantoprazole or other PPIs, the possibility of gastric or esophageal cancer must be ruled out because the drug may mask symptoms and delay cancer diagnosis.
- In patients with severe liver impairment, consider reducing the dose of Ulsepan 40mg or using it every other day; you can take 40mg every other day.
- Doses above 40mg/day have not been studied in patients with liver impairment; liver enzymes must be monitored during treatment. If an increase is observed, the drug should be discontinued immediately.
- Dose adjustment is not recommended in patients with renal impairment; however, the maximum oral dose should be 40mg.
- Gastrointestinal infections caused by bacteria increase the resident bacteria in the gastrointestinal tract in the stomach.
- Proton pump inhibitors are associated with very rare cases of subacute cutaneous lupus erythematosus. If lesions occur, especially in sun-exposed skin areas, and if accompanied by joint pain, the patient should see a doctor and consider discontinuing the medication.
