Ultrasound Shows No Pregnancy but Endometrial Thickness of 14mm – Could I Be Pregnant?

Question

Dear Doctor,

I have polycystic ovary syndrome (PCOS) and have been receiving treatment from a local doctor for the past month. I recently took a pregnancy test, which showed one dark line and one faint line. However, when I tested again the following day, only one line appeared. I then underwent an ultrasound, which did not detect a pregnancy, but my endometrial thickness was measured at 14mm. Does this mean I could be pregnant despite the ultrasound results?

Answer

Dear Ms. Như,

Regarding your question, "Ultrasound Shows No Pregnancy but Endometrial Thickness of 14mm – Could I Be Pregnant?", we would like to provide the following explanation:

Based on the information you have provided, there are two possible scenarios: Early pregnancy, in which the gestational sac has not yet implanted in the uterine cavity. Impending menstruation (your period is about to start). In both cases, the endometrium can be thickened. However, differentiation between these conditions can be made through blood testing to measure serum beta-human chorionic gonadotropin (β-hCG) levels.

  1. Impending menstruation: Multiple pregnancy tests yield negative results (only one line appears), ultrasound shows endometrial thickening but no gestational sac in the uterus, and β-hCG levels remain within the normal non-pregnant range.
  2. Early pregnancy: A pregnancy test may show one dark line and one faint line, ultrasound reveals a thickened endometrium without a visible gestational sac, and blood β-hCG levels are slightly elevated. If pregnancy is suspected, a follow-up assessment after one week is recommended: If pregnancy is progressing, the pregnancy test should show two dark lines, ultrasound should detect a small gestational sac (a few millimeters in size) within the uterine cavity, and β-hCG levels should rise significantly compared to the previous test.
