Tobradex 5ml is commonly used to treat eye inflammation and eye infections. During the use of Tobradex, patients need to follow the doctor’s instructions regarding dosage and frequency of application to ensure safety and achieve the best results.
1. What is Tobradex 5ml?
Tobradex 5ml is an eye drop suspension used to treat eye inflammation responsive to steroids in cases of superficial bacterial infections or a high risk of bacterial eye infections. Tobradex is a product of S.A. Alcon - Couvreur N.V., formulated as a sterile off-white suspension with the following specific active ingredient concentrations:
Active ingredients: Tobramycin 3mg (0.3%) and dexamethasone 1mg (0.1%).
Preservative: Benzalkonium chloride (0.01%).
Other excipients: Tyloxapol, disodium edetate, sodium chloride, hydroxyethyl cellulose, sodium sulfate, sulfuric acid and/or sodium hydroxide (to adjust pH), and purified water.
2. Indications and Uses of Tobradex
Tobradex 5ml is commonly indicated in the following cases:
- Treats steroid-responsive eye inflammation
- Treats blepharitis, keratitis, conjunctivitis, and anterior uveitis.
- Reduces swelling, pain, irritation, redness, and edema.
- Treats chronic anterior uveitis, injuries from foreign bodies, chemicals, radiation, or thermal burns.
- Treats high-risk superficial eye infections or large bacterial presence.
Additionally, the tobramycin and dexamethasone components in Tobradex 5ml exhibit activity against bacteria responsible for the following eye conditions:
- Streptococci strains: Certain non-hemolytic species, beta-hemolytic group A species, and Streptococcus pneumoniae.
- Staphylococci strains: Including S. epidermidis, S. aureus, and penicillin-resistant strains.
- Escherichia coli, Haemophilus influenzae, Pseudomonas aeruginosa, Proteus vulgaris, Acinetobacter calcoaceticus, and Neisseria.
3. Dosage and Administration
3.1 Dosage
- Below are the general recommended doses for using Tobradex:
1-2 drops in each eye (including non-inflamed eye) 4-6 times/day. - In the first 24-48 hours, the dosage may increase to 1-2 drops every 2 hours. Gradually reduce the frequency as symptoms improve. Avoid abrupt discontinuation.
- Tobradex eye ointment can be used at night instead of the eye drop suspension.
- Children: Tobradex can be used in children aged 2 years and older at the same dosage as adults. Available data are described in the Pharmacodynamic Properties section. The safety and efficacy in children under 2 years of age have not been established, and no data are available.
- Patients with hepatic or renal impairment: Tobradex has not been studied in these patient groups. However, dose adjustment is not required since tobramycin and dexamethasone have minimal systemic absorption when used in the eyes.
3.2 How to Use
- For eye drops only.
- After opening the cap, if the safety ring detaches, remove it before using the medication.
- Shake the bottle well before use.
- To avoid contamination of the dropper tip and the suspension, ensure that the tip does not touch the eyelids, surrounding areas, or any other surfaces.
- Close your eyes gently and press on the tear duct after administering the eye drops. This can help reduce systemic absorption of the medication through the eyes and minimize systemic side effects.
- If using other eye drops concurrently, ensure there is a 5-minute interval between applications. Eye ointments should always be used last.
3.3 What to Do in Case of Overdose or Missed Dose of Tobradex 5ml
Managing an Overdose of Tobradex 5ml
In the event of an overdose of Tobradex 5ml eye drops, the patient may experience symptoms such as redness, spots, swelling, excessive tearing, and eyelid itching. To address this, immediately rinse the eyes with clean, warm water to help remove the active ingredients. For severe overdose reactions, promptly contact a doctor or visit a healthcare facility for treatment.
Managing a Missed Dose of Tobradex 5ml
If a dose of Tobradex is missed, administer it as soon as possible. If it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to make up for the missed one, as this may increase the risk of an overdose.
4. Possible Side Effects of Using Tobradex 5ml
When using Tobradex, patients may experience some of the following common side effects:
- Itching or discomfort in the eyes
- Eye pain, increased intraocular pressure, eye irritation
- Eye allergies, blurred vision, red eyes, conjunctival redness
- Ocular congestion, eyelid swelling, eyelid redness, pupil dilation
- Increased tearing or sensitivity to light
- Secondary infection
- Taste changes

- Abdominal discomfort
If any of the above symptoms occur, promptly inform your doctor. These side effects can vary in severity from person to person, ranging from mild to severe. Therefore, monitoring the body's reactions after using the medication is crucial to ensure timely management of any potential health risks.
5. Contraindications for Using Tobradex 5ml
Avoid using Tobradex in the following cases:
- Individuals with a history of allergy to active ingredients such as tobramycin, dexamethasone, or hypersensitivity to any of the excipients in the medication.

- Individuals with Mycobacterium infections in the eye.
- Patients with fungal infections in ocular structures or parts of the eye.
- Untreated purulent eye infections.
During prolonged treatment with steroid medications, be aware of the risk of corneal fungal infections. In cases of superinfection, prompt management is necessary.
6. Precautions When Using Tobradex 5ml
6.1 What Should Be Noted During Treatment with Tobradex?
Tobradex eye drops should remain sterile until the cap is opened. Avoid contamination of the medication and do not use Tobradex if there are unusual signs, such as discoloration, cloudiness, or mold. In addition, you also need to take the followings into account:
- Do not inject Tobradex into the eyes.
- If hypersensitivity reactions such as redness or itching occur, discontinue use immediately.
- Tobradex contains dexamethasone (a corticosteroid) that can suppress the immune system, increasing the risk of fungal, viral, and bacterial infections and masking disease symptoms.
- Avoid wearing contact lenses while using Tobradex, as it contains benzalkonium chloride, which may irritate the eyes.
6.2 Drug Interactions
When using Tobradex, exercise caution with other medications to prevent harmful interactions. Avoid combining Tobradex with neurotoxic, nephrotoxic, or ototoxic agents, beta-lactam antibiotics, or neuromuscular blocking drugs.
To minimize the risk of drug interactions, provide your doctor with a complete list of medications you are taking, including herbal remedies, prescription drugs, over-the-counter medications, and health supplements. Avoid discontinuing or adjusting the dosage of Tobradex without consulting your pharmacist or doctor.
6.3 Special patient group for Tobradex 5ml
Pregnant Women:
There is insufficient research and evidence to confirm the safety of Tobradex for pregnant women. Use should only occur if prescribed by a doctor, after evaluating that the benefits outweigh the potential risks to the fetus.
Breastfeeding Women
It is unclear whether Tobradex is excreted in breast milk. However, studies have shown that tobramycin and corticosteroids can be excreted in breast milk when taken systemically. The potential risk to nursing infants cannot be excluded. Consider the benefits of breastfeeding against the benefits of treatment for the mother when deciding whether to stop breastfeeding or discontinue the medication.
Machine Operators or Drivers
Temporary blurred vision or other visual disturbances may impair the ability to drive or operate machinery. If blurred vision occurs after using the drops, wait until vision clears before driving or operating machinery.
6.4 How to Store Tobradex 5ml
Store at a temperature not exceeding 30°C. Keep the bottle upright.
Tobradex 5ml is commonly used to treat eye inflammation and bacterial infections. During treatment, patients must follow their doctor’s instructions regarding dosage and frequency of use to ensure safety and achieve optimal results.
