Ankle joint pain refers to swelling in the ankle accompanied by pain and limited mobility. Although this condition is not considered dangerous, patients still need professional advice and treatment under the guidance of a specialist.

1. What is ankle joint pain?
The ankle joint connects the lower leg and foot and consists of bones, cartilage, synovial fluid, blood vessels, nerves, tendons, and ligaments. Ankle joint pain can be a symptom of various conditions such as rheumatoid arthritis, ankle osteoarthritis, lupus erythematosus, etc.
Early detection of ankle joint pain is important, whether the pain is mild or severe. However, the condition can be fully treated and recurrence prevented if combined with proper musculoskeletal care and a healthy lifestyle.
2. Causes of ankle joint pain without swelling
The ankle joint is composed of several bones including the tibia, fibula, talus, calcaneus, cuboid, navicular, and metatarsals, all stabilized by a system of ligaments and tendons.
As a highly flexible joint that enables walking, the ankle often bears significant pressure from body weight during standing.
Many factors can cause ankle joint pain, including injury, sprain, tendinitis, ankle arthritis, osteoarthritis, and tarsal tunnel syndrome. Causes like injury, ligament strain, and ankle arthritis are often accompanied by swelling and pain.
2.1 Tarsal Tunnel Syndrome
Tarsal tunnel syndrome occurs when ligaments or nerves around the ankle are compressed, leading to chronic pain. People at high risk include dancers, high jumpers, and volleyball players.
Constant bending and stretching of the ankle while walking can compress nerves and tendons. Other symptoms may include joint stiffness, numbness, or tingling in the foot. However, some patients may also experience ankle swelling with pain.
2.2 Ankle Osteoarthritis
Ankle osteoarthritis commonly affects people over 40 and those with previous ankle injuries. The disease progresses slowly and is often difficult to detect early due to unclear symptoms. However, as the condition worsens, patients may feel increased pain and discomfort during movement.
Patients may experience morning stiffness that eases with activity.
Sharp pain can occur suddenly, especially during strenuous activity or when pressing or bumping the affected joint. The pain intensity ranges from mild to severe, worsening with movement and easing with rest.
When experiencing pain, patients should limit ankle joint movement. Prolonged movement restriction can lead to muscle atrophy, joint stiffness, and in some cases, ankle joint deformity.
Ankle osteoarthritis can cause symptoms such as swelling, warmth, redness, and increasing pain in the ankle and foot. Patients may have joint effusion and persistent pain throughout the day.
Although ankle osteoarthritis is not life-threatening, it can seriously impact quality of life and mobility.
Possible complications of ankle osteoarthritis include:
- Persistent pain: When cartilage deteriorates, underlying bone is exposed, causing severe pain during movement. Bone spurs may also develop, pressing on surrounding nerves and tendons, causing pain in other body parts.
- Muscle atrophy and lower limb disability: Untreated pain can lead to irreversible joint deformity, muscle wasting, paralysis, disability, and loss of mobility.
- Other complications: Joint deformity, dislocation, abnormal gait, and cartilage fragments may block the joint or damage surrounding soft tissues.
3. Should you see a doctor if you have ankle joint pain without swelling?
If a patient experiences ankle joint pain without swelling, they should see a doctor immediately to determine the cause and receive timely and appropriate treatment.
